羟基磷灰石植入眼窝成形术不同术式随访观察

摘　　要：目的：观察羟基磷灰石植入眼窝成形术不同手术方法的临床效果。方法：对５０例接受羟基磷灰石植入眼窝成形术患者的临床和随访资料进行回顾性研究；对不同手术方法的近期和远期效果进行比较分析。结果：５０例患者中施行一期眼内容剜出自体巩膜覆盖羟基磷灰石眼窝内植入术２６例（５２％）；一期眼球摘除异体巩膜覆盖羟基磷灰石眼窝内植入术５例（１０％）；二期眼内容剜出自体巩膜覆盖羟基磷灰石眼窝内植入术４例（８％）；二期眼球摘除异体巩膜覆盖羟基磷灰石眼窝内植入术１５例（３０％）。接受随访时间平均为２８．３个月。随访并发症有轻度眼窝凹陷２例；结膜囊轻度狭窄２例；义眼台转动功能不良１６例；术后３０天内结膜切口裂开８例，其中６例经修补后愈合，另外２例自行愈合。结论：不同条件下应采用不同术式的羟基磷灰石眼窝内植入眼窝成形术；一期植入者并发症少。OBJECTIVE: To evaluate clinic effect of various socket plastic technique on application of hydroxyapatite implant. METHODS: Retrospective study was performed according to clinic and followup data of 50 patients in which hydroxyapatite were used as implants of socket plastic technique. Postoperative and followup clinic effect on various socket plastic technique was compared and analyzed. RESULTS: There were twenty six cases (52%) of primary evisceration and hydroxyapatite implants covered with autoscleras , five cases (10%) of primary enucleations of eyeball and hydroxyapatite implants covered with alloscleras, four cases ( 8% ) of secondary evisceration and hydroxyapatite implants covered with autoscleras, and fifteen cases ( 30% ) of secondary enucleations of eyeball and hydroxyapatite implants covered with alloscleras, among the 50 patients. The average of follow-up period was 283 months. Two of 50 patients had slight concave socket, two had slight strictures of conjunctival sac, sixteen had undesirable motility of implants, eight had postoperative dehiscence of conjunctival incision in 30 days but 6 of them cicatrized with secondary surgery and 2 cases were spontaneous cured in the complications observed. CONCLUSION: Various types of socket anaplasty for hydroxyapatite implants could perform according to different condition of patients. There were a few complications and good prosthesis motility in the primary implants.
