血管内皮细胞生长因子-C在乳腺癌肿瘤相关巨噬细胞内的表达及临床意义

摘　　要：目的研究乳腺癌组织中的肿瘤相关巨噬细胞计数(TAM)及血管内皮细胞生长因子-C(VEGF-C)在TAM中的表达,探讨其临床病理意义。方法观察乳腺癌组织免疫组织化学双重染色镜下表现,并检测75例乳腺癌组织中TAM计数及TAM中的VEGF-C的表达率与临床病理因素间的关系。结果免疫组织化学双重染色显示:VEGF-C阳性表达的TAM中可见在紫黑色胞浆内有深红色颗粒存在,且VEGF-C在肿瘤内呈弱阳性表达,而在肿瘤周围间质的TAM中表达呈强阳性。在乳腺癌组织中TAM计数以及TAM中VEGF-C表达率随临床分期(0-Ⅰ期、Ⅱ期和Ⅲ-Ⅴ期)递增,且三者之间比较,差异均有统计学意义(F=24.56、28.64,P均<0.05)。淋巴结无转移病例中TAM计数和TAM中VEGF-C表达率明显低于淋巴结转移病例,雌激素阳性病例中TAM计数低于阴性病例,差异均有统计学意义(t分别=2.99、2.66、2.55,P均<0.05)。TAM计数与TAM中VEGF-C蛋白表达率之间呈正相关(r=0.95,P<0.05)。结论TAM计数及TAM中VEGF-C表达率与反映乳腺癌进展、淋巴结转移和预后有关。Objective To investigate the counts of tumor-associated macrophages （TAM）, expression of vascular endothelial growth factor-C （VEGF-C） in TAM and the relationship with clinical features in breast carcinoma. Methods Microscopic appearance in breast carcinoma was observed by immunohistochemical double-staining technique. Relationship between the counts of TAM, expression of VEGF-C in TAM and clinical features were detected. Results The experiment of immunohistochemical double-staining showed there were cardinal red granules in violet cytoplasm of VEGF-C positive TAM, and VEGF-C was weakly positive in intratumoral tissue, and strongly positive in peritumoral tissue. The counts of TAM in breast cancer as well as the expression of VEGF-C in TAM increased by clinical stage （0-Ⅰ stage, Ⅱ stage and Ⅲ-Ⅴ stage）, and had a significant difference （F=24.56, 28.64, P〈0.05）. The counts of TAM and the expression of VEGF-C in TAM of cases without lymph node metastasis were significantly lower than that of lymph node metastasis. The counts of TAM in cases of estrogen-positive was lower than that of ER-negative （t=2.99, 2.66, 2.55, P〈0.05）. There was positive correlation between the counts of TAM and the expression of VEGF-C in TAM（r=0.95, P〈0.05）. Conclusions The counts of TAM and the expression of VEGFC in TAM may be the important biological indicators to reflect the progress of breast cancer, lymph node metastasis and prognosis.
