摘 要:目的观察慢性砷中毒的多焦视网膜电图变化,探讨其发病特点。方法采用VERIS Science^(TM)5.2多焦电生理系统对18例(36眼)慢性砷中毒病例进行多焦视网膜电图(mERG)检测,并选年龄和性别与之相匹配的正常人18例(36眼)作为正常对照组进行同样检测。为了观察病情变化分别在就诊时、治疗21d,60d、90d进行mERG检测,比较视网膜不同区域特征峰的反应密度和潜伏期。结果慢性砷中毒患者在就诊时与正常对照组之间比较,第1环N1波和P1波潜伏期无统计学差异,振幅密度轻度下降。第2~6环显示N1波和P1波的振幅密度下降和潜伏期明显延长(P<0.01),并随着病情发展,各环均与正常对照组出现统计学差异,同时发现多焦视网膜电图改变甚至早于视力下降等临床表现的出现。结论慢性砷中毒后在早期就会出现视网膜的不可逆损害,mERG具有特征性改变,对于疾病的早期诊断和监测病情变化具有独特的价值。Objective To explore the use of multifocal electrorefinograms (mERG) in detecting changes in chronic arsenic poisoning. Methods 18 patients who were diagnosed chronic arsenic poisoning underwent mERG testing in both eyes using RETI Scan multifocal ERG (Version 5.2), 18 ages matched normal peoples were included as controls. In order to observe changes in condition, they were tested in 21, 60, 90 days. The first order kernel responses of mERG were recorded and to analyze in six concentric rings centred on the fovea. Results There were no significant differences in the amplitudes and the latencies between the 1 ring regions when compared with the normal control group. There was a significant difference in N1 wave amplitude and a significant difference in the P1 wave amplitude between the two groups at ring 2 - 6 ( P 〈 0.01 ). And with the changes condition, the ring appears significant difference compared with normal controls. We also found that mERG changes even earlier than clinical manifestations. Conclusions Chronic arsenic poisoning will appear irreversible damage to the retina in the earlier mobility. The mERG is a useful test for clinical diagnosis and monitoring of disease change.
