摘 要:目的探讨预缺血对长时间缺血/再灌注心肌的保护作用。方法在氨基甲酸乙酯麻醉家兔心肌模型上,观察缺血/再灌注组和预缺血组对血流动力学、心功能、心肌耗氧量、心外膜电图和心肌梗塞范围的影响,并对结果进行统计分析。结果两组在缺血/再灌注过程中,血流动力学,心功能,心肌耗氧量均明显持续性降低,且两组间心肌耗氧量存在明显差异;心外膜电图ST段在缺血过程中明显抬高;缺血/再灌注组和预缺血组的心肌梗塞范围分别为32.3%和21.6%,有明显差异(P<0.01)。结论预缺血可降低心肌耗氧率,减轻心肌损伤。Objective To research the protection on myocardium with more prolonged ischemia-reperfusion provided by ischemic preconditioning in urethane-anesthetizied rabbits.Methods The effects of ischemia-reperfusion group and ischemic preconditioning group on the hemodynamics, cardic function, myocardial oxygen consumption ,epicardial electrographic,infarct size were examined in urethane-anesthetized rabbit models.the results were analysized by statistic method.Results In the course of ischemia(30min)/reperfusion(180min),all hemodynamic parameters,cardiac function and myocardial oxygen consumption were decreased progressively significantly, while myocardial oxygen consumption in the both groups showed difference(P<0.05).The epicardial electrogrophic ST-segment during myocardial ischemia were markedly increased.The myocardial infarct size of the left ventrical in ischemia-reperfusion group and ischemic preconditioning group were 32.2% and 21.6% respectively, its had significant difference(P<0.01).Conclusion Ischemic preconditioning could decrease myocardial oxygen consumption and reduce injury of myocardium and it provides the cardioprotection during ischemia-reperfusion.
