颈动脉粥样硬化和血脂水平与脑梗塞的相关研究

摘　　要：【目的】探讨颈动脉粥样硬化及血脂水平与脑梗塞之间的关系。【方法】对102例脑梗塞患者(脑梗塞组)及82例非脑梗塞患者(对照组),分别进行颈动脉超声检查,并记录两组颈动脉粥样硬化斑块的部位、数目和颈动脉内径,同时检测两组患者的血脂水平。【结果】脑梗塞组颈动脉粥样硬化斑块检出率为76.47%,对照组为26.83%,两组间比较差异有统计学意义(P<0.01)。脑梗塞组左右颈总动脉内径分别为(7.41±0.08)mm、(7.49±0.61)mm,与对照组(7.76±0.09)mm、(7.98±0.74)mm比较差异均有显著性(P均小于0.05);斑块部位与脑梗塞部位有显著同侧相关性(P<0.05);脑梗塞组甘油三脂(2.05±0.19)mmol/L也显著高于对照组(1.38±0.18)mmol/L(P<0.05)。【结论】颈动脉粥样硬化斑块与脑梗塞发生有密切关系,部分脂质代谢异常与颈动脉粥样硬化斑块形成有关。[Objective] To investigate the relationship between carotis artherosclerosis and blood-fat level with cerebral infarction. [Methods] Color ultrasonic inspection of carotid artery was taken in 102 patients with cerebral infarction （CI group） and 82 patients without cerebral infarction（control group）. Position, number of artherosclerotic plaques and inner diameter of carotid artery were recorded. Blood-fat level of each patient at the same time. [Results] The detection rate of artherosclerotic plaque was 76.47% in CI group and 26.83% in control group. The difference was statistical significance（P 〈 0.01）. Inner diameter of common carotid artery of both side in CI group were （7.41 ±0.08） mm and （7.49 ±0.61） mam, which are （7.76±0.09） mm,（7.98 ±0.74） mm in control group. The differences are all statistical significance （P 〈 0.05）. The position of plaque had significant homonymy assoeiativity with the position of cerebral infarction （P 〈 0.05）. the triglyceride（TG） of CI group was （2.05 ±0.19） mmol/L, which was notably higher than control group （1.38 ±0.18） mmol/L （P 〈 0.05）. [Conclusions] There is a close relationship between carotid artherosclerotic plaques and cerebral infarction. Abnormal lipid metabolism is related with the formation of carotid artherosclerosis plaques.
