摘 要:目的探讨组织因子(TF)、组织因子途径抑制剂(TFPI)-1、2在乳腺癌转移中的作用。方法106例乳腺癌患者血液和组织标本,根据肿瘤大小、淋巴结转移情况、TNM分期分别分组。同时设立健康对照组。酶链免疫吸附试验(ELISA)法检测血浆TF、TFPI-1、TFPI-2浓度;免疫组织化学染色观察TF、TFPI-1、TF-PI-2的组织表达。结果乳腺癌组TF以及TFPI-1血浆浓度均高于对照组[对照组TF浓度(0.63±0.24)U/L,TFPI-1浓度(80±19)μg/L];TFPI-2各组比较差异无统计学意义。TF血浆浓度在乳腺癌>2cm组为(1.76±0.42)U/L,较<2cm组(0.83±0.24)U/L增高;淋巴结转移阳性组较淋巴结转移阴性组高2倍;Ⅲ/Ⅳ期(1.8±0.4)U/L高于Ⅰ/Ⅱ期(0.9±0.3)U/L(P<0.05)。Ⅲ/Ⅳ期TFPI-2血浆浓度(3.0±1.2)μg/L显著低于Ⅰ/Ⅱ期(5.1±1.2)μg/L(P<0.05)。TF在乳腺癌>2cm组及淋巴结转移阳性组表达平均秩次分别为150.09、157.01,分别较<2cm组(111.23)、淋巴结转移阴性组(109.16)高表达;Ⅲ/Ⅳ期(162.58)较Ⅰ/Ⅱ期(112.47)表达增强(P<0.05)。ⅢI/Ⅳ期乳腺癌TFPI-2表达则较Ⅰ/Ⅱ期降低,2组表达平均秩次分别为59.92、98.94;>2cm组(50.97)表达<2cm组(94.37);TFPI-1血浆浓度及组织表达在乳腺癌各组差异无统计学意义(P>0.05)。结论乳腺癌高表达TF,TF与乳腺癌肿瘤直径、淋巴结转移及其TNM分期有关,其高表达参与了乳腺癌的生长和转移。TFPI-1在肿瘤生长和浸润中的作用尚不能肯定。TFPI-2在乳腺癌细胞的表达与肿瘤的生长与转移关系密切,其低表达与乳腺癌的进展相关。Objective To evaluate the effect of tissue factor (TF), tissue factor pathway inhibitor-1,2(TF- PI-1,2) on breast cancer. Methods To collect blood and tissue samples from 106 breast cancer patients and 130 healthy person as control. To divide cancer samples into different teams according to cancer size, node metastases and TNM stages. Then, to measure TF, TFPI-1 and TFPI-2 plasma concentrations by ELISA; to observe tissue expression of TF, TFPI-1 and TFPI-2 by immuno-histochemical method. Results Plasma concentrations of TF and TFPI-1 in breast cancer are higher than those in the control; TF plasma concentrations of 〉2 cm team, node positive team and Ⅲ/Ⅳ stage are higher than that of 〈2 cm team, node negative team and Ⅰ / Ⅱ stage respectively. TFPI-1 has no statistical differences. TFPI-2 of Ⅲ -Ⅳ stage is lower than Ⅰ /Ⅱ stage. Tissue expression of TF of breast cancer is higher than the adjacent tissues. TF expression in 〉2 cm team, node positive team and Ⅲ/ Ⅳ stage are higher than that in 〈2 cm team, node negative team and Ⅰ / Ⅱ stage respectively. There is no difference for TFPI-1 expression. TFPI-2 expression in Ⅲ/Ⅳ stage and 〉2cm team are lower than that in Ⅰ /Ⅱ stage and 〈2 cm team respectively. Conclusion TF expression of breast cancer is related to cancer size, node metastases and TNM stages, higher TF expression takes part in breast cancer growing and metastases. TFPI-1 has no clear effect on cancer growing and metastases. Expression of TFPI-2 relates to breast cancer growing and metasta- sis. Lower expression of TFPI-2 relates to advanced stages of breast cancer.
