河北省邢台市2002～2006年饮食服务从业人员实验室检查项目与频次研究

摘　　要：[目的]调查研究中华人民共和国从业人员预防性健康检查实验室检查项目与频次,为我国制定切实可行的检查项目与频次提供科学依据。[方法]采用连续监测速率法测定血清中ALT活性,ELISA方法检测血清中HBV5项,对阳性者进行走访调查。大便培养采用统一的培养基和检验方法,按操作常规进行培养和检测。[结果]ALT活性升高并非都是HBV感染,而HBV感染不一定ALT活性升高;HBsAg阳性为新聘用的初检人员,在5年的续检人员中,HBsAg全是阴性。但是,HBsAg阴性不能排除HBV感染;大便培养检测痢疾、伤寒或副伤寒全是阴性。[结论]ALT检测对预防HBV传染没有现实意义,建议从实验室检查项目中删去;为防止隐匿性HBV遗漏,建议检测HBV5项;对抗-HBs阴性者,检测的间隔时间不少于2年;对抗-HBs阳性者,续检的间隔时间不少于5年,但不应超过10年;一年一次的大便培养检测,建议从实验室检查项目中删去,归为有碍公共卫生疾病的其他类。[Objective] To investigate and study the practical staff preventive laboratory checkout program examination project and the frequency in China,so as to provide scientific basis for formulate the practical and feasible preventive automatic system checkout program examination project and the frequency in our country.[Methods] Use the continuous monitoring speed law determination blood serum ALT activity.And double antibody-sandwiched enzyme linked immunoassay assay ELISA was used for detection of HBsAg and HbeAg.The practical staff who being infected were followed up.[Results] The ALT activity raises not for all HBV infectionj,the HBV infection also not showed ALT activity raises.The people with HBsAg positive were newly hired personnel.In the five years health examines,the HBsAg presented to be negative.[Conclusion] The examination project of ALT has not real meaning in the protection of liver infectious disease.So the ALT inspection should obliterate from the examination project.In order to prevent invisible positive HBV,the HBeAg health check-up should be changed to detect the five targets of HBV.For people with negtive of HBsAg and-HBs,the check-up showed not be more than two years,for people who has the positive of-HBs should check-up at least for five years,but not exceeded ten years.The defecation checking items should delete from the preventing health check-up for practical staff,and could be imputed to other health checking items.
