B型超声对胰腺转移性肿瘤的诊断及分析

摘　　要：回顾分析１５例经手术病理证实的胰腺转移性肿瘤的超声诊断资料。结果表明转移性胰腺肿瘤Ｂ超表现为弱回声，边界清，形态欠规则。与原发胰腺癌不同，胰腺转移瘤较少引起胆管及胰管扩张，认为可能与本组转移性肿块多来自胃癌的直接侵犯，或通过淋巴转移致胰腺周围淋巴结，肿块自被膜方向向内生长有关。胰腺转移癌与原发胰腺癌的鉴别诊断存在一定困难。The sonograms of 15 cases with pancreatic metastasis were analysed retrospectively. The diagnosis was verified by pathology. On sonogram, pancreatic metastases usually showed as hypoechoic foci with well defined margin and irregular shape. The pancreatic and bile duct were seldomly dilated in contrast to primary pancreatic carcinoma. The authors suggested the possible mechanism is that most of the metastatic tumors in this group were caused by direct invasion of gastric cancer, or lymphatic metastasis to peripancreatic lymph nodes. The tumors growde from outside to inside. It is somewhat difficult to differentiate metastatic from primary pancreatic tumors without preexisting primary lesion and has to combine with clinic history.
