不同剂量沙利度胺对多发性骨髓瘤的疗效观察

摘　　要：目的观察不同剂量沙利度胺联合化疗方案(VAD)治疗多发性骨髓瘤(MM)的临床疗效及不良反应。方法23例患者随机分为二组,分别以沙利度胺400～800 mg/d+VAD(A组)及沙利度胺50～200 mg/d+VAD(B组)治疗。结果A组组部分缓解4例,进步2例,无效2例,总有效率75%。B组部分缓解6例,进步5例,无效4例,总有效率73.3%。副反应:皮疹、便秘、嗜睡、周围神经病变、深静脉栓塞的发生A组均明显高于B组。结论沙利度胺的剂量<200 mg/d较为合适,既可得到相应的疗效又可降低副反应。Objective To observe the effects and the side effects with different doses of thalidomide combine chemotherapy regimen （VAD） in treating multiple myeloma patients. Method We divided 23 patients with multiple myeloma into two groups randomly.The Group A received treatment with 400-800mg/d of thalidomide combine the VAD regimen. And the Group B is treated with 50-200mg/d of thalidomide combine the VAD regimen.Result The group A：part remission （PR）4 pts, progress 2 pts, therapy-nave 2 pts,ORg 75%.The group B： PR 6 pts, progress 5pts,therapy-nave 4 pts,ORR 73.3 % .The incidence rate of drug side effects in group A is apparently higher than in group B such as erythra,oppilation,hyp ersom- nia,peripheral neuropathy and venous profundae embolism.Conclusion To treat with the dose of thalidomide below 200mg/d can receive a same effects and reduce the side effects.
