吉首市社区空巢老人缺血性卒中一级预防状况研究

摘　　要：目的了解吉首市城市社区空巢老人缺血性卒中一级预防及缺血性卒中危险度分布状况。方法以美国心脏协会/美国卒中协会《缺血性卒中的一级预防指南(2006年修订本)》和《中国脑血管病防治指南——危险因素干预治疗建议(综合表)》所载明确的可干预的危险因素为基础,自行设计《吉首市城市社区空巢老人缺血性卒中一级预防状况调查问卷》。2009年4—7月间,采取整群抽样的方式对吉首市向阳社区、荣昌社区和峒河社区389名空巢老人进行入户问卷调查。结果向阳社区、荣昌社区、峒河社区空巢老人缺血性卒中危险度筛查评分分别为(53.7±7.2)、(57.5±10.7)、(51.9±6.4)分;缺血性脑卒中极高危和高危者所占比例分别为78.4%、69.3%、77.9%。本组空巢老人缺血性卒中4项核心危险因素——高血压病、糖尿病、心房颤动及其他心脏病、血脂异常获得有效控制者分别为36.6%、26.5%、44.4%、35.1%。结论吉首市城市社区空巢老人缺血性卒中极高危和高危者所占比例大,高血压病、糖尿病、心房颤动及其他心脏病、血脂异常4项核心危险因素控制不够理想。Objective To understand the primary prevention and risk factors distribution of ischemic stroke among community empty nest seniors in Jishou. Methods Data was collected from April to July of 2009 by cluster sampling among 389 empty nest seniors in Xiangyang, Rongchang, and Donghe communities of Jishou by self-designed questionnaire based on ＂Primary prevention of ischemic stroke： a guideline from the American Heart Association/American Stroke Association Stroke Council （Edition 2006）＂ and ＂Guideline for China＇s Prevention and Treatment of Cerebral Vascular Disease＂. Results The scores on screening ischemic stroke among empty nest seniors in Xiangyang, Rongchang, and Donghe communities were （53.7±7.2）, （57.5±10.7）, and （51.9±6.4） respectively; the rate of empty nest seniors classified into extremely high-risk group or high-risk group of ischemic stroke were 78.4%, 69.3%, and 77.9%, respectively; the rate of effective control of hypertension, diabetes, atrial fibrillation and other heart diseases, and lipid abnormality were 36.6%, 26.5%, 44.4%, and 35.1%, respectively. Conclusion The rate of ischemic stroke risk of empty nest seniors in extremely high-risk group or in high-risk group are high in Jishou communities; and key risk factors of ischemic stroke, including hypertension, diabetes, atrial fibrillation and other heart diseases, and lipid abnormality, are under poor control.
