临床路径在白内障盲防治中的应用研究

摘　　要：目的通过对临床路径在白内障盲防治应用中的实际效果分析,探索符合我国国情和眼科实际的白内障临床路径设计、组织和实施的办法,评价临床路径在白内障盲防治中的实际效果。方法对我院眼科2006年10月～2009年4月间住院行白内障(单眼)手术的病例临床随机分组,按照责任医师组将患者分为传统组和临床路径组,临床路径组128例按照临床路径的诊疗流程进行处理,传统组325例则按照传统的诊疗程序实施诊疗。比较两组在住院日、住院费用、并发症及满意度方面的差异。结果白内障临床路径实施后,使平均住院日由3.13天缩短到2.04天,缩短了1.09天;平均住院费用由6972.11元降低到6580.91元,减少了391.20元。临床路径组的住院日、住院费用均少于传统组,差异显著(P<0.05)。临床路径组的患者好评度均为100%。临床路径组与传统组并发症发生率无显著差异。结论临床路径在白内障盲防治中能显著地提高医疗护理质量,降低医疗成本,减少医疗资源的消耗,让更多白内障盲患者受益。Objective By analysing the actual effect for the application of clinical pathway in preventing and control- ling of cataract blind, to search the way fit the situation of our country and the application in ophthalmology. Methods The clinical pathway group（128 cases carried on processing according to the clinical pathway diagnosing and treating flow, the tradi- tional group （325 cases） refer to traditional diagnosing and treating procdure implementation diagnosing and treating. Compared length of stay, hospital costs, complication and degree of satisfaction of two groups. Results The clinical pathway groupJs length of stay and hospital costs were short to the traditional group,the difference obviously （P〈0.05）. The cliniclal pathway group/s patient high praise was 100%. Clinical pathway group and traditional group complication at formation rate was not re- markable difference. Conclusion The clinical pathway can obviously improve the medical care quality in cataract blind pre- venting and controlling, reduces the medical cost, reduces the consumption of the medical resources, benefit more cataract blind patient.
