摘 要:目的探讨计算机辅助检测(computer-aided detection)在多层CT结肠成像(CT colonography)中对结肠息肉诊断的应用价值。方法采用GE64层螺旋CT对23例高度怀疑结肠息肉患者进行仰卧位和俯卧位快速容积薄层扫描后行CT结肠成像。经纤维结肠镜活检证实共60个息肉,不分病理类型,对息肉大小进行分组,直径≤5mm10个,直径6-9mm21个,直径≥10mm29个。3位对纤维结肠镜结果不知情的读片者在无/有计算机辅助检测帮助下对23例患者CT结肠成像结果进行诊断(至少两个诊断者诊断相同的结果视为最终结果),对不同组息肉检出的敏感度进行比较分析。结果在计算机辅助检测帮助下对10个直径≤5mm的结肠息肉检出敏感度由40%提高到60%,P<0.05;21个直径在6-9mm的结肠息肉检出敏感度由67%提高到86%,P<0.05;29个直径≥10mm的结肠息肉患者的检出敏感度在无/有计算机辅助检测帮助下分别为90%和93%,P>0.05。结论在多层CT结肠成像中应用计算机辅助检测(computer-aideddetection)可明显提高对直径小于10mm的结肠息肉检出的敏感度,对6-9mm范围内的息肉敏感度提高了19%,直径≤5mm结肠息肉检出敏感度由40%提高到60%,提高了20%。Objective To explore the value of computer-aided detection in multi-layered CT colonography in colon polyp diagno- sis. Methods CTC ( with patients in both supine and prone positions) was performed with GE 64-MD CT scarmer in 23 patients high- ly suspected with colon palps. Sixty colon polyps were confirmed by colonoscope, of them ten was smaller than or equal to 5 rnm, twen- ty-one was 6 - 9 mm and twenty-nine was greater than or equal to 10 mm in diameter. Three readers blinded to the result of fibro- colonoscope read the result without and then with computer-aided detection. The diagnostic result would be confirmed at least two of the three readers gave the same diagnosis The sensitivity of detection polyps without and with computer aided detection was assessed. Results With CAD as a second reader,the sensitivity of detecting polyps smaller than or equal to 5 mm in diameter increased from 40% to 60%, P 〈 0.05;it increased from 67% to 86% for 21 colon polyps between 6 - 9 mm in diameter, P 〈 0.05;sensitivity of detecting bigger than or equal to 10mm polyps increased from 90% to 93 %, P 〉 O. 05. Conclusion Computer aided detection is helpful in the multi-layered CT eolonography for detecting polyps smaller than lOrnm in diameter.
