经尿道诊断性膀胱肿瘤电切在膀胱癌分期诊断中的作用

摘　　要：目的探讨经尿道诊断性膀胱肿瘤电切在膀胱癌准确分期的临床应用价值。方法回顾性分析我院在1995年3月至2005年3月期间收治的行经尿道膀胱肿瘤电切术(经典术式)的199例膀胱癌患者,与在2006年5月-2009年1月期间收治的行经尿道诊断性膀胱肿瘤电切术的82例膀胱癌患者临床随访资料。结果①在行经尿道膀胱肿瘤电切术的膀胱癌患者中,术后两内年膀胱癌原位复发6例,术后5年、7年膀胱内无肿瘤复发,而出现盆腔淋巴结转移及全身骨转移2例。②在行经尿道诊断性膀胱肿瘤电切的膀胱癌患者中,有8例证实肿瘤基底部肌层可见癌组织浸润,根据患者个人一般状况,采取膀胱全切或其他治疗方案。结论经尿道诊断性膀胱肿瘤电切术能够准确的对膀胱肿瘤进行分期,对指导膀胱癌治疗提供准确的依据。Objective To evaluate the clinical application value in accurately staging of bladder tumor by the diagnostic transurethral resection of bladder tumor. Methods One hundred and ninty-nine patients＇ were treated by transurethral resection of bladder tumor（TURB）from Mar. 1995 to Mar. 2005 and eighty-two cases which were treated by diagnostic TURB from May. 2006 to Jan.2009 in our hospital were reviewed. Results Six patients who received TURB recurre bladder tumor in situ during two years, while 2 cases emerge the pelvic lymph node metastasis and systemic bone metastasis after 5 years and 7 years non-bladder tumor re- currence. Eight cases were confirmed muscule invasine by the diagnostic TURB, and followed by bladder radical cystectomy or others treatment. Conclusion the Diagnostic TURB cart accurately stage the bladder tumor and provide accurate guiding to the treatment of bladder tumor.
