前牵引矫治安氏Ⅲ类错疗效的Meta分析

摘　　要：[目的]探讨前牵引矫治安氏Ⅲ类错时对上下颌骨及上颌前牙的作用,此分析排除了自然生长因素的影响,从而更真实地反映该类矫治器的矫治功能。[方法]收集发表有关用上颌的前方牵引器矫治安氏Ⅲ类错的文献,运用Meta分析对其治疗前后SNA、SNB、Wits、Mx1-NA的变化进行综合定量分析。根据资料的异质性检验,均采用随机效应模型,计算合并WMD及其95%CI。[结果]①资料的异质性检验:投影测量指标SNA、SNB.wits异质性检验I2≥50%,具有异质性。②Meta分析结果:SNA、SNB、Wits、Mx1-NA的并WND均不包括0,结果均有统计学意义,分别是2.02(1.70,2.64),-1.49(-1.78,-1.19),4.51(3.35,5.66),0.57(0.37,0.76)。[结论]前方牵引器增加了上颌基骨的长度,使上颌骨和上颌牙前移,抑制下颌骨向前发育,下颌骨发生顺时针旋转,从而患者侧貌明显改善,前牙反合得以矫治。[Objective] To evaluate the maxillary,mandibular and the anterior teeth changes produced by the protraction appliance excluding the influence of natural growth,thereby,it is more objective,actual and accurate to reflect the treatment effect.[Methods] Meta-analysis was used to make formal summary statements about the effect of protraction appliance used in the mixed dentition with cephalometric analysis such as SNA,SNB,Wits,Mx1-NA.According to homogeneity test,all factors adopted random effect model.Random effect model should be selected to calculate total pooling WMD and its corresponding 95% confidence interval（95%CI）.[Results] ① Homogeneity test showed that of SNA,SNB and wits（I2≥50%） were significant heterogeneity.② Result of meta-analysis：the total pooled WMD value（95%CI） of SNA,SNB,Wits,Mx1-NA did not in-clude 0,which was 2.02（1.70,2.64）,-1.49（-1.78,-1.19）,4.51（3.35,5.66）,0.57（0.37,0.76） respectively,and all showed significant statistical difference.[Conclusion] Maxillary protraction therapy significantly increases the maxillary length and inhabits mandibular growth so the significant changes take place in the patients＇ profile.
