乳腺癌保乳手术和乳房再造术的疗效和生活质量对照研究

摘　　要：[目的]评价乳腺癌保乳手术和乳房再造术的疗效和生活质量。[方法]随访180例乳腺癌患者一般社会学特征、临床病例资料、术后情况。分为保乳组(n=90)和再造组(n=90)两组。180例患者获得随访,随访期1～7年。比较了两组1年、3年、5年、7年的生存率,保乳组分别为100%、92.77%、80.16%、63.57%。再造组分别为100%、92.86%、87.75%、66.50%,Log-rank检验结果示(P﹥0.05)。两组在局部复发率和远处转移率比较差异无统计学意义(P﹥0.05)。应用(FACT-B4.0)量表对120例入选患者的生活质量进行测试,结果显示:在生理功能、社会功能、躯体功能、性功能、身体形象以及QOL总分方面,保乳组好于再造组,但差异无统计学意义(P﹥0.05)。心理功能方面再造组平均得分高于保乳组,两组比较差异有统计学意义(P﹤0.05)。[结果]实行保留乳房手术的患者和实行乳房再造术患者疗效和生活质量一致。[Objective] To assess the efficacy and QOL of the breast-conserving surgery and the real-time breast reconstruction after modified radical mastectomy in breast cancer patients.[Methods] Of 180 total cases,90 cases of breast cancer were treated with breast-conserving surgery（BC group） and 90 cases of breast cancer were treated with the real-time breast reconstruction after modified radical mastectomy（MR group）.Their sociodemographic,clinical characteristics and situations after surgery were followed up.180 cases were followed up from 1 year to 7 years,the 1-,3-,5-,7-year survival rate of two groups were 100%,92.77%,80.16%,63.57% and 100%,92.86%,87.75%,66.50%.Log-rank test showed there was no significant difference（P ﹥ 0.05）.There was no significant difference（P ﹥ 0.05） between local recurrence rate and distant metastasis rate.QOL was evaluated on the 120 cases by using FACT-B Questionnaire Chinese version 4.0.Results showed BC group was better than MR group on the dimension of physical function,social function,functional adjustment,sexual function,body image and total of QOL.But the difference was not significant（P﹥0.05）.The QOL was assessed on psychologic function.Results showed MR group was better than BC group on the dimension of psychologic functioning QOL（P﹤0.05）.[Conclusion] Patients with breast cancer who are treated with breast-conserving surgery and the real-time breast reconstruction after modified radical mastectomy have the same efficacy and QOL.
