摘 要:目的:探讨血清多肿瘤标志物蛋白芯片检测系统在结直肠癌诊断中的价值。方法:分析200例结直肠癌和100例结直肠良性疾病患者的多肿瘤标志物蛋白芯片检测系统检测结果。结果:结直肠癌组血清CEA、CA19-9、CA242、CA125水平和阳性率明显高于结直肠良性疾病组(P<0.05),结肠癌患者血清CA242水平明显高于直肠癌患者(P<0.05),但阳性率无统计学差异(P>0.05),Ⅰ期+Ⅱ期结直肠癌患者血清CEA、CA19-9、CA242、CA125水平和阳性率明显低于Ⅲ期+Ⅳ期结直肠癌患者(P<0.05),联合检测CEA、CA19-9、CA242、CA125的敏感性、准确性和诊断指数均高于单项检测(P<0.05),但特异性之间的差异无统计学意义(P>0.05)。结论:血清肿瘤标志物检测对结直肠癌有肯定的诊断价值,可作为肿瘤分期的一个重要依据。联合检测可以显著提高结直肠癌诊断的敏感性和准确性,提高早期检出率,CEA+CA19-9+CA242组合可作为诊断结直肠癌的最优化组合。Objective: To explore the diagnostic value of muti-tumor markers protein biochip system in colorectal cancer. Methods: The serum concentrations of 12 tumor markers (CA199, NSE, CEA, CA242, Ferrtin, β-HCG, AFP, f-PSA, PSA, CA125, HGH, and CA153) in 200 patients with colorectal cancer and 100 patients with benign colorectal lesions were detected and analyzed by C12 multi-tumor markers protein chip diagnosis system. Results: The expression levels and positive rates of the serum tumor markers, such as CEA, CA19-9, CA242, and CA125 in patients with colorectal cancer were significantly higher than those respectively in patients with benign colorectal lesions (all P〈0.05). The expression level of serum CA242 in patients with colon cancer was significantly higher than that in patients with rectal cancer (P〈0.05), but the posi tive rate had no statistical significance (P〉0.05). The expression levels and positive rates of serum CEA, CA19 9, CA242, and CA125 in staging Ⅲ and IV colorectal cancer patients were significantly higher than in staging I and II patients respectively (all P〈0.05). The sensitivity, accuracy, and diagnosis index of combined detection of CEA, CA19-9, CA242, and CA125 were higher than those of single detection respectively (all P〈0.05), while no statistical significance was found between the specificity of single detection and the specificity of combined detection (P 〉0.05). Conclusion: The detection of serum tumor markers had positive value in the diagnosis of colorectal cancer. Combined detection could improve the diagnostic sensitivity and accuracy significantly and the combined detection of CEA, CA19-9, and CA242 is the most beneficial combination among the 12 markers in the diagnosis of colorectal cancer.
