摘　　要：目的探讨应用结肠移植食管重建术治疗胃大部切除术后中段以上食管癌的疗效。方法回顾性分析1999年至2006年期间,胃大部切除术后患中段以上食管癌并接受结肠移植食管重建术治疗的18例患者病历资料。结果 18例接受结肠移植食管重建术治疗患者均获手术成功。术后1例并发声音嘶哑,2例并发颈部吻合口瘘,3例并发肺部感染,总并发症发生率为33.3%(6/18),除1例术后肺内重症感染患者因呼吸衰竭围手术期死亡外,其余5例出现术后并发症患者均治愈。结论胃大部切除术后中段以上食管癌采用结肠移植食管重建术治疗是可行、安全的,为减少术后并发症应加强围手术期的处理和精细手术操作。Objective To investigate the therapeutic efficacies using colon transplantation and esophageal reconstruction in the treatment of esophageal cancer of above middle segment after subtotal gastrectomy. Methods From January 1999 to December 2006, 18 cases with above middle segment esophageal cancer after subtotal gastrectomy received colon transplantation and esophageal reconstruction were analyzed retrospectively. Results The operations were performed successfully in all cases. Postoperative complications were found in 6 cases, including hoarseness in 1 case, anastomotic fistula in 2 cases, pneumonia in 3 cases. Five cases with postoperative complications were cured, and one case died from severe pneumonia associated with respiratory failure. Conclusion In the treatment of above middle segment esophageal cancer after subtotal gastrectomy, the use of colon transplantation and esophageal reconstruction is feasible and safe. For reducing postoperative complications,an enhancing preoDerative management and nrecise onerative maninulation are verv imnortant medium.
