喉部分切除术治疗喉癌效果观察

摘　　要：目的:观察喉部分切除术治疗喉癌的效果。方法:对11例喉癌采用不同术式的喉部分切除,其中行喉裂开声带切除1例,喉额侧垂直部分切除1例,喉垂直部分切除7例,喉扩大垂直部分切除术2例,3例行分区性颈清扫术,1例术后辅以放射治疗。结果:术后生存满5年者7例,满3年者3例,满1年以上Ⅲ期病例死于肺部转移1例。全部吞咽功能恢复良好,无误咽;10例在正常时间拔管,1例放疗后拔管,无误吸;全部患者保存了发音功能,8例较清晰,3例声音低粗、嘶哑。结论:手术效果满意,达到了切除肿瘤、保留功能,提高病人生活质量的目的。Objective： To observe the effects on treatment of laryngocarcinoma patients after partial laryngectomy. Methods 11 patients were treated by partial laryngectomy for different operations. 1 case with Laryngofissure cordectomy, 1 case with vertical partial frontal- laryngectomy, 7 cases with vertical partial laryngectomy, 2 cases with extended vertical partial laryngectomy, 3 cases with selective neck dissection, 1 case with postoperative adjuvant radiotherapy. Results 5 years Survivor were 7, 3 years Survivor were 3, 1 case of Ⅲ stages over 1 year died of lung metastasis. All of these patients regained their swallowing function well, no one with erroneous deglutition; 10 cases extubated in normal time, 1 case extubated after adjuvant radiotherapy without aspiration; All of these patients preserved pronunciation function, among which 8 cases were comparatively clear, 3 cases voice were lower, made and hoarseness. Conclusion The effect of laryngocarcincma with partial laryngectomy is satisfaction and reach the purpose to resect the tumor, reserve the function and improve the living quality of patients.
