正畸联合自体牙移植治疗前牙埋伏阻生的临床研究

摘　　要：目的:观察正畸联合自体牙移植术治疗前牙埋伏阻生的临床效果并探讨其适应证。方法:临床就诊患者20例,年龄8～17岁,平均12岁。阻生部位以尖牙最多,所有患者均先行正畸治疗以拓展间隙,之后行手术将牙齿移植于合适位置,不损伤牙周膜及牙乳头,牙根周围骨缺损区可植入适量羟基磷灰石(HA),钢丝或夹板固定。术后3～6个月进一步正畸治疗。结果:20例患者术后随访2～5年移植牙均稳固于牙列中,牙龈形态良好;X线片追踪观察,移植牙牙根无吸收现象。结论:正畸联合自体牙移植术治疗前牙埋伏阻生,可增进美观,恢复牙弓外形及口腔功能,为一种可行的治疗方法。Objective To explore the clinical effects and indications of treating the impacted anterior teeth by tooth autotransplantation combined with orthodontics. Methods 20 patients with impacted anterior teeth were reported in this paper.Most of impacted teeth are cuspid.All patients were firstly treated by orthodontics to expand the space, followed by teeth autotransplantation.The periodontal membrane and dental papilla must be protected well.After 3-6 months,further orthodontic treatment can be carried on. Results 20 patients were followed-up 2-5 years.All teeth were aligned into the dentition and with a good gum forms. Root absorption can not be observed from X-ray films. Conclusions The combined treatment of tooth autotransplantation and orthodontics can restore the dental arch form and oral function,could be an effective way to treat complicated impacted anterior teeth.
