全牙列垫治疗颞下颌关节痛的临床疗效观察

摘　　要：目的研究全牙列垫治疗颞下颌关节痛的临床效果。方法选取颞下颌关节疼痛患者分为治疗组和对照组,治疗组360例,其中低龄组(小于35岁)194例,高龄组(大于或等于35岁)166例,接受全牙列垫治疗;对照组100例,接受健康宣教。分别于治疗(或宣教)前和治疗(或宣教)后2周、2个月、3个月对2组患者进行视觉模拟尺(visual analog scale,VAS)疼痛评价。结果戴入垫后1～2周,治疗组患者的疼痛、弹响、张口受限等关节紊乱症状均有缓解。全牙列垫组和对照组疗效差异有统计学意义(χ2=297.11,P<0.05)。低龄组治疗后3个月的VAS评分低于高龄组,差异有统计学意义(t=5.159,P<0.05)。结论全牙列垫能有效缓解颞下颌关节痛患者的临床症状,低龄组患者垫治疗的远期效果好于高龄组。Objective The aim of this clinical observation was to investigate the clinical therapeutic effect of the whole dentition occlusal splint on the patient with the derangement of temporomandibular joint. Methods A total of 360 patients,who reported having suffered from temporomandibular joint disorders and serious tooth attrition,were interviewed. A kind of soft film made by Dental Resources Inc. was chosen,with different thickness. Before using of the occlusal splint,we adjusted the cusps of teeth in the dentition,choosed suitable soft film and then made it into the comfortable shape wanted. The patients were asked to use them everyday and get further consultation regularly. Results Most of the patients relieved partially from all the symptoms of the temporomandibular joint disorders such as pain,snap and restriction of mouth opening after using the occlusal splint about 2 weeks later,some of them relieved completely two to three months later. The suffers＇questionnaire evaluation before and after treatment had differences as a result （P〈0.05）. The limited degree of deference and no-prompt restoration could lead to the defeat of treatment in some patients. Conclusion The whole dentition occlusal splint could restore condyles in the suitable functional place of glenoid through the way to rectify the positon of mandible correspondent maxillary,and then patients could establish a new balance among muscle,joint and jaw positon,eliminate premature contact and occlusal interference in addition.
