食管鳞癌术后复发因素的Logistic回归分析

摘　　要：目的探讨影响食管鳞癌术后复发的相关因素,为降低食管癌治疗后复发的方法提供依据。方法应用SPSS13.0软件对52例食管鳞癌患者的复发情况与14个可能的影响因素进行χ2单因素分析和Logistic多元回归分析。结果单因素分析显示T分期、N分期、p53基因的表达、survivin基因在癌周组织中的纵向表达范围、术后是否进行50Gy以上放疗等5个因素与肿瘤复发有关。多因素研究发现T分期、survivin基因在癌周组织中的纵向表达范围和手术后是否接受50Gy以上剂量放疗是3个影响食管癌复发的独立因素。结论T分期中T3、T4期患者复发的风险高;手术后接受50Gy以上剂量放疗能明显降低肿瘤的复发率,survivin基因在癌周组织中的表达范围是影响食管癌复发的独立因素。Objective To study the influential factors of recurrence after surgical opertion of esophageal squaraa carcinoma（ESC）, provide evidences for diminishing the probability of recurrence. Methods Analysis the relation between 14 factors and recurrence by SPSS 13. 0 sofeware. Univariate analysis was performed using a Chi-Square test and multivariate analysis was performed using logistic regression modeling. Results Univariate analysis revealed a significant relationship between recurrence and 5 factors： T stage, N stage, the positive expression rate of p53 gene, the positive expression ranges of survivin gene in cancerous peripheral tissues,whether or not finished or exceed 50 Gy radiotherapy after operation. Multivariate analysis showed that 3 factors were independent influencing factors： T stage, the positive expression ranges of survivin gene in cancerous peripheral tissues, whether or not finished or exceed 50 Gy ra- diotherapy after operation. Conclusion Patients with T3, T4 stage had even higher risk for the recurrence of ESC. Patients finished or exceed 50Gy radiotherapy after operation had lower recurrence rate. The positive expression ranges of survivin gene in cancerous peripheral tissues were another independent influential factor.
