摘 要:目的观察银杏达莫注射液是否对兔缺血后适应心肌保护作用具有协同强化效应,并对其机制进行探讨。方法48只新西兰大白兔随机分成4组,每组12只。行开胸手术,结扎冠状动脉左前降支根部行30min缺血/3h再灌注,制作缺血/再灌注模型(对照组);再灌注时予3次10s复流/10s闭合操作复制缺血后适应模型(PC组)。实验前3d,每日给予家兔经颈静脉注射银杏达莫注射液1mL/kg,行开胸手术,结扎冠状动脉左前降支根部行30min缺血/3h再灌注,制作缺血/再灌注模型(银杏达莫组);再灌注时予3次10s复流/10s闭合操作复制缺血后适应模型(联合组)。每组随机抽取6只测定心肌梗死面积及血清肌酸激酶(CK)、髓过氧化物酶(MPO)、丙二醛(MDA)、超氧化物歧化酶(SOD)、一氧化氮合酶(NOS)含量;其余6只测定心肌超微结构的变化,并通过免疫组化方法观察对心脏血红素氧化酶-1(HO-1)蛋白表达的影响。结果与PC组及银杏达莫组相比,联合组可以进一步缩小心肌梗死面积,降低血清再灌注1、2、3hCK值,降低MPO、MDA活性,升高SOD、NOS活性,可以显著改善心肌超微结构损伤,并可上调心肌HO-1的表达。结论缺血后适应联合应用银杏达莫注射液,可以产生协同作用,加强缺血后适应对心肌的保护作用,并且HO-1在心肌的高表达可能参与了其心肌保护作用机制。Objective To investigate if postconditioning(PC) together with yinxingdamo injection has coordinated protection of the myocardium from severe ischemia/reperfusion.Methods 48 Newsland rabbits were divided randomly into four groups (n=12).The control group underwent left coronary artery (LCA) occlusion for 30 min followed by 3 h of reperfusion;PC group were subjected to circulation and occlusion respectively for 10 seconds on alternate basis when given reperfusion.Yinxingdamo group was pretreated with yinxingdamo injection [1 mL/(kg·d)]for 3 days before the experiment,and no intervention was administered after LCA occlusion;Combination group underwent the procedures performed in both PC group and Yinxingdamo group.6 rabbits of each group were randomly selected,whose plasma CK,MPO,MDA,SOD,and NOS levels were detected,and the infarct area was measured.For the rest 6 rabbits of each group,the influence of PC on myocardium ultrastructure the expression of H0-1 in the hearts and were observed.Results compared with PC and yinxingdamo group,combination group the infarct size reduced;the plasma CK activity at the onset,1 h,2 h,3 h of reperfusion lowered;the plasma MPO,MDA activity lowered and the plasma SOD,NOS activity ascended;myocardial ultrastructure got improved;the expression of HO-1 in heart was upregulated.Conclusion PC together with yinxingdamo injection could protect the myocardium from severe I/R injury in coordination;It suggests that high expression of HO-1 is involved in cardioprotection mechanism of PC and yinxingdamo injection.
