旋复代赭汤对食管运动功能紊乱的临床观察

摘　　要：目的：观察旋复代赭汤对食管运动功能紊乱，胃食管反流病的临床疗效。方法：采用CTD仪检测187例有食管病变临床表现患者，监测24／小时食管的pH值变化，对食管上，中、下段分别测压，将其中表现为胃食管反流病，食管运动功能紊乱及食管各段动力正常的患者随机分成2组，分别用旋复代赭汤与西药普瑞博斯加泰胃美片口服作对照，观察2组患者各用药一个月后的临床变化及结果。结果：旋复代赭汤对食管运动功能紊乱有明显双向调节作用。LES低压型失松弛型，EB远段高幅蠕动或低相蠕动型，UES高压型或低压型的患者多数获得临床缓解，有效率达74．36％，疗效近似西药组；对食管反流病患者的有效率仅71．43％，明显低于西药组（西药组有效率88．89％）；对有明显临床症状，CTD检测结果正常的患者有效率达100％，远远优于西药组（西药组有效率23．08％）。结论：旋复代赭汤对食管运动功能紊乱有廊好双向调节作用。Observing the clinical therapeutic effect of the treatment of esophageal motility disorder and gestroesophageal reflux with Inulae Haematifum decoction. Methods: 187 patients withesophageal diseases were monitoredby esophageal PH for 24 hours. The pressure at the upperesophageal sphincter, lower esophageal sphincter and in the esophagus was measured by intraesophageal monometer. After this examination the patients were divided into three test groups: 1 )patients with gastroesophageal reflux; 2)patients with esophageal motility disorder; 3)patients withnormal esophageal motility. There 3 groups were each divided into 2 groups again. One group ofeach test group was treated with Inulae haematifum decoction. The control groups were treatedwith Tagamet and Cisapride. The outcome and clinical curative effect were observed after onemonth of treatment. Results:Inulae Hematifum decoction has an obvious effect in the treatment ofesophageal motility and gastroesophadeal reflux in most patients, who suffered,from loweresophageal sphincter pressure or achalasia;high amplitude peristalsis or low amplitude peristalisisof the esophagus;high or low upper esophageal sphincter pressure,the rate of effective remisson ofsymptoms was 74. 3%, similar to the control groups. In the treatment of gastroesophageal refluxthe rate was 71. 43 %, lower than in the control groups with 88. 89 %. In the treatment of patientswith obvious symptoms and normal esophageal motility the effective rate of Inulae Haematifum decoction is 100%, obviously higher than in the control groups with 23. 08%. Conclusion: InulaeHematifum decoction has an obvious effect on treating esophageal motility disorder and gastroesophageal reflux.
