胃癌患者血清胃动素,肽YY,IgG和铁蛋白变化的意义

摘　　要：目的探讨胃癌患者血清胃动素（ＭＴＬ），肽ＹＹ（ＰＹＹ），免疫球蛋白Ｇ（ＩｇＧ）及铁蛋白（ＦＲＴ）含量的变化及临床意义．方法胃癌患者２８例，男１９例，女９例，平均４８６岁（２５岁～６１岁），均经纤维内镜，Ｘ线钡餐和组织病理学检查确诊，并排除肠道肿瘤和内分泌疾病．病变按胃段分布包括胃窦部１７例，胃底部５例，胃体部６例；按胃壁分布包括胃小弯１８例，胃大弯４例，前壁２例和后壁３例；按胃癌的病期分为早期４例和中晚期２４例；按组织学分为腺癌１７例，粘液腺癌５例，低分化腺癌３例和未分化癌３例．健康对照２６例，男１７例，女９例，平均４６４岁（２３岁～５９岁），排除胃肠道和内分泌疾病．全部研究对象均于确诊后３ｄ内的上午０７：００空腹采血，用放射免疫法测定血清ＭＴＬ，ＰＹＹ，ＩｇＧ和ＦＲＴ．结果与对照比较，胃癌患者血清ＭＴＬ（ｎｇ／Ｌ，２０７３±９８４ｖｓ３１６３±９８４，Ｐ＜００１），ＰＹＹ（ｐｍｏｌ／Ｌ，１１±０６ｖｓ１７±０８，Ｐ＜００１）和ＩｇＧ（ｇ／Ｌ，１３１±７０ｖｓ２５３±１０１，Ｐ＜００１）明显升高，而ＦＲＴ（μｇ／Ｌ，９８４±４１５ｖｓ１２７１±６４１，Ｐ＞００５）?AIM To determine the change of serum levels of motilin (MTL), peptide YY (PYY), immunoglobulin G (IgG) and ferritin (FRT) in patients with gastric cancer, and their clinical significance. METHODS A total of 28 patients with gastric cancer consisting of 19 men and 9 women with a mean age of 48 6 years,including the antrum ( n =17), the body ( n =6), and the fundus ( n =5). Twenly six healthy normal subjects consisting of 17 men and 9 women with a mean age of 46 4 years was studied as controls. Samples of fasting blood were collected in dry tubes. Serum levels of MTL, PYY, IgG and FRT were measured with radioimmunoassay kit. RESULTS In patients with gastric cancer, serum MTL ( ng/L , 207 3±98 4 vs 316 3±98 4, P <0 01), PYY ( pmol/L ,1 1±0 6 vs 1 7±0 8, P <0 01) and IgG ( g/L , 13 1±7 0 vs 25 3±10 1, P <0 01) were much higher, and FRT ( μg/L , 98 4±41 5 vs 127 1±64 1, P >0 05) was unchanged,as compared with normal subjects. The serum MTL level was related with the PYY ( r =0 4436, P <0 05).In Post operative 15 patients, serum IgG ( g/L , 16 4±7 3 vs 26 6±10 1, P <0 05) was significantly lowered, and MTL, PYY and FRT had no change, compared with preoperative ones. CONCLUSION The serum levels of motilin, peptide YY, immunoglobulin G and ferritin are increased in patients with gastric cancer, and have a clinical significance for study of gastric cancer.
