基质金属蛋白酶-2和基质金属蛋白酶-9在宫颈上皮内瘤样变和宫颈癌组织中的表达

摘　　要：目的探讨从宫颈癌前病变进展至宫颈癌,基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)的表达变化。方法采用免疫组织化学二步法检测20例慢性宫颈炎、45例宫颈上皮内瘤样变(CIN)、42例宫颈癌中MMP-2和MMP-9蛋白的表达。结果MMP-2和MMP-9在慢性宫颈炎中的表达极低,在各级CIN、宫颈浸润癌阳性表达高,其阳性表达强度随CIN级别增高而增强,至宫颈癌表达强度最高,并差异有显著性;宫颈癌的临床分期与MMP-9的表达强度有显著性关系,但MMP-2的表达强度无显著性关系。结论MMP-2、MMP-9在宫颈癌前病变进展至宫颈癌这一过程中起着重要作用,而MMP-9与宫颈癌进展的关系更为密切,成为判断宫颈癌预后的指标之一。【Objective】To evaluate the level of matrix metalloproteinase-2 （MMP-2） and matrix metalloproteinase-9 （MMP-9） in chronic cervicitis, the precursor lesions （cervical intraepithelial neoplasia, CIN） and the invasive carcinoma.【Methods】We evaluate samples of cervical tissues using immunohistochemistry for MMP-2 and MMP-9 in 5 distinct groups. The groups were divided into chronic cervicitis, CIN Ⅰ , CIN Ⅱ , CIN Ⅲ and cervical invasive carcinoma. 【Results】MMP-2 and MMP-9 expression was found gradually increased according to the degree of CIN and cervical carcinoma. （chronic cervicitis〈CIN I 〈CIN II 〈CIN Ill 〈carcinoma）. The expression degree of MMP-9 was correlated with the clinical stage of carcinoma, but the expression degree of MMP-2 was not. [ Conclusions ] MMP-2 and MMP-9 play important role in the process of CIN developing to carcinoma, but MMP-9 is more important according to the development of carcinoma, which might be a useful prognostic indicator for cervical cancer.
