Lasik术后两种糖皮质激素应用方案的临床观察

摘　　要：目的探讨一种安全、有效的糖皮质激素用药方法。方法采用前瞻性研究(双盲),观察Lasik治疗的100例(200眼)患者,根据术后用药不同分为试验组、对照组,各100眼。其中试验组Lasik术后使用典必殊眼液10d,对照组使用氟米龙眼液和托百士眼液10d,两组患者除术后糖皮质激素应用不同外,术前准备,手术方法及术后其他用药均相同。观察指标包括术前1d,术后1d,术后10d,术后1个月,术后3个月的眼部症状(包括眼干、异物感)及体征,主要为眼压及弥散性角膜基质炎,角膜荧光素染色。等级资料采用非参数独立样本U检验,计量资料采用计量资料统计学方法。结果术后10d试验组和对照组DLK差异有统计学意义(P<0.05),角膜荧光素染色试验组在1个月后恢复术前水平,对照组在3个月时仍未恢复术前水平,而其他观察指标各时间段差异均无统计学意义(P>0.05)。结论Lasik术后短期使用典必殊安全、方便,可获得预期的临床效果。Objective To explore a safe and efficient way of using glucocorticoid. Methods prospective study （doubleblind） was applied and 100 patients （200 eyes）underwent Lasik were chose randomly and divided to experimental group and control group according to different medication after operation, with 100 eyes in each group. The patients in experimental group used TobraDex for 10 days and the control group were applied with Flumetholon and Tobrex for 10 days after Lasik. Except that different glucocorticoid was used ,there were no difference of preparation before operation, operating method and other eye drops between two groups. The subjective complaints including dryness of eye and foreign body sensation,intra-ocular pressure, diffused lamellar keratitis（DLK） and corneal fluorescence staining were oberserved 1 day preoperatively and 1 day, 10 day, 1 month and 3 months postoperatively. Ranked data was analyzed by u test of nonparameter independent sample and measurement data was analyzed by corresponding statistical method. Results The DLK has signifficant difference statistically on day 10 after operation between two groups. Corneal fluorescence staining in experimental group recovered to the preoperative level on month 1 （ P〈0. 05 ） , but the level of fluorescence staining in control was still higher on month 3 than before. There were no significant difference for other observing index in the period of experiment （ P〉0. 05 ）. Conclusion It was safe and convenient to using at short term after Lasik and prospective therapeutic effect would be received.
