胃癌及癌前病变中人胎盘型谷胱甘肽S转移酶的表达和意义

摘　　要：目的：通过检测人胎盘型谷胱甘肽Ｓ转移酶（ＧＳＴπ）在胃癌和胃癌前病变组织中的表达情况，研究胃癌与ＧＳＴπ之间的关系。方法：用Ｓ－Ｐ法免疫组化技术检测了ＧＳＴπ在５８例胃癌、２５例胃粘膜肠上皮化生、２８例胃粘膜不典型增生和３９例正常胃粘膜组织中的表达情况。结果：胃癌、胃粘膜肠上皮化生和胃粘膜不典型增生组织中ＧＳＴπ表达阳性率依次为７７６％（４５／５８）、７６０％（１９／２５）和８９３％（２５／２８），而正常胃粘膜组织仅１０３％（４／３９）呈微弱阳性。在胃癌标本中，高分化腺癌、低分化腺癌、粘液腺癌和印戒细胞癌的阳性率依次为８３３％（１０／１２）、８９３％（２５／２８）、６６７％（６／９）、２５０％（１／４）和６００％（３／５）。可见ＧＳＴπ在胃癌及癌前病变组织中的表达较正常胃粘膜高（Ｐ＜００１）；分化较好的腺癌高于分化较差的腺癌。结论：ＧＳＴπ有胃癌及癌前病变中的表达与胃癌的发生发展密切相关。Aims: To detect the expression of the human placental form of glutathione S transferase (GST π) in human gastric carcinoma and precancerous lesion,and to study the relationship between gastric carcinoma and GST π Methods: GST π was detected in human gastric carcinoma,precancerous lesion and normal gastric mucosas with immunohistochemical technique (S P) method.Results: It was shown that 77 6%(45/58) of gastric carcinoma,76.0%(19/25) of intestinal metaplasia and 89.3%(25/28) of atypical hyperplasia were positively stained,while most of 39 normal gastric mucosas were negative,with only 4(10.3%) weakly stained.The positive rate of well differentiated adenocarcinoma,poorly differentiated adenocarcinoma,undifferentiated adenocarcinoma and signet ring cell carcinoma were 83.3% (10/12),89.3%(25/28),66.7%(6/9),25.0%(1/4),60.0%(3/5) respectively The expression of GST π in gastric carcinoma and precancerous lesion was markedly higher than that in normal gastric mucosas (P<0 01), and there was more expression of GST π in the higher differentiated degrees of gastric carcinoma,as compared to the lower differentiated degrees of gastric carcinoma (P<0 05).Conclusions: It was indicated that GST π is involved in carcinogenesis and development of gastric carcinoma and is a useful marker for immunohistochemical detection of gastric carcinoma and precancerous lesion.
