摘 要:目的分析运动神经元疾病(MND)患者血清和脑脊液中神经节苷脂(GM1)抗体的分布,研究其在MND发病过程中的作用。方法采用ELISA方法检测了45例不同类型的MND患者血清和脑脊液中IgMGM1、IgGGM1抗体的水平。结果MND患者血清中IgMGM1、IgGGM1抗体均明显高于对照组,其中IgMGM1抗体在进行性脊肌萎缩症患者中阳性率最高,而IgGGM1抗体在MND的不同临床类型中差异无显著性意义。MND患者脑脊液中IgMGM1和IgGGM1抗体也均明显高于对照组(P<0.01),在MND不同临床类型中差异无显著性意义(P>0.05)。MND患者脑脊液中IgMGM1和IgGGM1抗体与血清中IgMGM1和IgGGM1抗体均不相关。结论MND患者周围和鞘内均存在对GM1的免疫反应,并可能参与了MND的发病过程。Objective To study the distribution of antibody against GM1 ganglioside in serum and CSF, and to probe into its pathogenetic role in motor neuron disease(MND). Method Using enzyme linked immunosorbent assays (ELISA), we examined the levels of serum and CSF antibody to GM1 Ganglioside in 45 patients with MND. Results The levels of serum IgM GM1 and IgG GM1 in the patients with MND were significantly higher than those in the normal controls( P <0.01). The frequency of IgM GM1 in the patients with progressive spinal muscular atrophy (PSMA) was significantly higher than that in the patients with other types of MND, but no difference for antibody to IgG GM1 in the patients with various types of MND( P >0.05). The results also indicated that the antibodies of IgM , IgG GM1 in CSF were not correlated to those in serum. Conclusion There are circulating and intrathecal humoral immune responses against GM1 in patients with MND. These humoral immune responses may play an important role in the pathogenesis of MND.
