摘  要:目的研究肾癌三维彩色血管能量成像(3D-CPA)及微血管密度(MVD)检测与肿瘤转移的关系。方法应用3D-CPA术前检测120例肾癌患者,进行血管分型,计算血管指数,检测术后肾癌肿块内MVD,并对肾癌患者按Robson法进行临床分期,分析MVD与其肿瘤转移的关系。结果3D-CPA血管分型肾癌Ⅰ期、Ⅱ期的以Ⅱ型、Ⅲ型血管为主,肾癌Ⅲ期、Ⅳ期以Ⅲ型血管为主。肾癌Ⅲ期、Ⅳ期的血管指数、MVD较Ⅰ期、Ⅱ期大(P<0.05)。肾癌3D-CPA血管分型、血管指数与MVD相一致,且肾癌血管指数与MVD呈正相关(r=0.842,P<0.01)。结论3D-CPA能客观显示各期肾癌的血管分布及数量,可作为肾癌分期诊断及评估转移预后的参考指标,将3D-CPA评价肾癌血管生成情况与MVD检测结合起来能更准确地评价肾癌的生物学特征。Objective To research the relationship between microvessel density (MVD) and three - dimensional color power angiography (3D -CPA)with tumor metastasis in patients with renal cell carcinoma(RCC). Methods One hundred and twenty patients with RCC were examined by 3D - CPA. Then the blood vessels were classified and vascularity index (Ⅵ) was calculated. The MVD in RCC tumors was detected after operation, and the patients with RCC were staged according to Robson method, the relationship between MVD and tumor metastasis was analyzed. Results There was significant difference of vessel patterns and VI of 3D-CPA in stage Ⅲ ,iv and stage Ⅰ , Ⅱ (P 〈0.01).The Ⅵ and MVD in stage 19 ,Ⅳ were more than those in stage Ⅰ , Ⅱ (P 〉 0.05). The results of vessel patterns and Ⅵ in 3D-CPA were coincident with MVD, and there was positive correlation between VI and MVD in patients with RCC (r = 0.842 ,P 〈 0.01 ). Conclusion 3D-CPA can show the number and distribution of blood vessels in different stages of RCC objectively, and it can become an important index to staging RCC and evaluate tumor metastasis. It can assess the biological characteristics of RCC more accurately by 3D - CPA combined with MVD.
