摘  要:目的分析散瞳前后中央角膜厚度(central corneal thickness,CCT)测量数值差异的原因。方法近视患者143例(286眼),随机分为A组、B组和C组,选用右眼CCT测量值进行观察。用超声角膜测厚仪测量CCT后,A组按常规滴用托吡卡胺闭眼散瞳;B组不滴用托吡卡胺仅闭眼;C组按常规滴用托吡卡胺后闭眼2min,然后睁开。60min后测量CCT,测量后,3组均正常睁眼15min后再测CCT,比较前后3次CCT测量数值。结果A组3次CCT测量结果分别为(535.11±36.40)μm、(548.50±35.91)μm、(538.09±36.17)μm,B组(531.09±31.93)μm、(543.61±31.53)μm、(533.41±31.84)μm,C组(524.84±30.32)μm、(525.56±30.29)μm、(525.08±30.34)μm;A组、B组第2次、第3次测量值均较第1次测量值高,差异均有显著统计学意义(均为P<0.001);C组3次CCT测量值比较均无明显变化,差异均无统计学意义(P>0.05)。3组间第2次测量值比较,A组和B组间差异无统计学意义(P>0.05);C组与A组、B组间差异均有统计学意义(均为P<0.05);第3次测量值3组间比较差异均无统计学意义(均为P>0.05)。结论闭眼导致缺氧是引起散瞳前后CCT变化的主要原因。Objective To investigate the causes for differences of central corneal thickness(CCT) before and after mydriasis.Methods A total of 143 myopia patients (286 eyes) were randomly divided into three groups,group A,group B and group C.CCT data of right eyes were observed.After measuring CCT with ultrasonic corneal thickness tester,patients were treated with mydriasis by tropicamide and closes eyes in group A,just closed eyes in group B,and just mydriasis by tropicamide in group C.CCT was measured 60 minutes later,and measured again after opening eyes for 15 minutes in three groups.Data of CCT were compared among three times.Results Data of three measurements were(535.11±36.40)μm,(548.50±35.91)μm,(538.09±36.17)μm in group A,(531.09±31.93)μm,(543.61±31.53)μm,(533.41±31.84)μm in group B,(524.84±30.32)μm,(525.56±30.29)μm,(525.08±30.34)μm in group C,respectively.The second and third CCT data of group A and group B were significantly higher than the first data(all P〈0.001);No significant difference was found among three times data of group C,neither at the second CCT data between group A and group B or at the third CCT data among three groups(all P〉0.05);While there was significant difference for group C compared with group A or group B at the third CCT data(both P〈0.05).Conclusion Hypoxia caused by closing eyes may be the main reason for CCT changes before and after mydriasis.
