摘  要:目的比较超声乳化白内障吸出术后植入Array多焦点人工晶状体和1cu可调节人工晶状体后的调节能力。方法26例(40眼)年龄相关性白内障患者行超声乳化白内障吸出及人工晶状体植入术,根据植入的人工晶状体的种类不同分为2组:Array组:14例20眼,植入Array多焦点人工晶状体;1cu组:12例20眼,植入1cu可调节人工晶状体。分别于术后3个月、18个月比较2组患者的最佳矫正远视力、35cm处最佳矫正远视力基础上获得的近视力及调节幅度。结果术后3个月时,2组患者的最佳矫正远视力、35cm处最佳矫正远视力基础上获得的近视力及调节幅度差异均无统计学意义(均为P>0.05)。术后18个月时,Array组患者的35cm处最佳矫正远视力基础上获得的近视力为0.36±0.12,优于1cu组的0.28±0.10,差异有统计学意义(t=2.29,P<0.05);Array组的调节幅度为(1.39±0.40)D,大于1cu组的(0.87±0.30)D,差异有显著统计学意义(t=4.65,P<0.001)。术后18个月时,Array组患者的35cm处最佳矫正远视力基础上获得的近视力及调节幅度与术后3个月(0.35±0.10,1.31D±0.50D)比较差异均无统计学意义(均为P>0.05);1cu组患者的35cm处最佳矫正远视力基础上获得的近视力及调节幅度与术后3个月(0.38±0.11,1.44D±0.40D)相比均出现一定程度的下降(均为P<0.05)。结论植入1cu可调节人工晶状体或Array多焦点人工晶状体均能为患者提供较好的近视力及调节能力,但植入1cu可调节人工晶状体患者的远期调节能力出现一定程度的下降。Objective To compare the accommodative power of Array multifocal intraocular lens(IOL) and 1cu accommodative IOL after phacoemulsification.Methods A total of 26 age related cataract patients(40 eyes) were treated with phacoemulsification and IOL implantation.According to the type of implanted IOL,all patients were divided into two groups,Array group(14 cases,20 eyes) implanting Array multifocal IOL and 1cu group(12 cases,20 eyes) implanting 1cu accommodative IOL.Best corrected visual acuity(BCVA) was compared between the two groups at postoperative 3 months,18 months.Near vision and accommodative amplitude at 35 cm away achieved on the basis of BCVA were also compared between the two groups.Results No significant difference was found in BCVA,near vision and accommodative amplitude at 35 cm away achieved on the basis of BCVA between the two groups at postoperative 3 months(all P〉0.05).Near vision at 35 cm away achieved on the basis of BCVA was 0.36±0.12 in Array group,better than 0.28±0.10 in 1cu group at postoperative 18 months,and there was statistical difference between the two groups(t=2.29,P〈0.05);The accommodative amplitude was (1.39±0.40)D in Array group,better than (0.87±0.30)D in 1cu group,and there was significant difference between the two groups(t=4.65,P〈0.001).Near vision and accommodative amplitude at 35 cm away achieved on the basis of BCVA were 0.35±0.10 and (1.31±0.50)D in Array group at postoperative 3 months,which had no significant difference with those at postoperative 18 months(both P〉0.05).Near vision and accommodative amplitude at 35 cm away achieved on the basis of BCVA were 0.38±0.11 and (1.44±0.40)D in 1cu group at postoperative 3 months,which were increased in some extent compared with those at postoperative 18 months(both P〈0.05).Conclusions Both implantation of 1cu accommodative IOL and Array multifocal IOL can help patients to get better near vision and accommodative power,but the accommodative power of patient
