外阴恶性肿瘤中P21、ILK及HPV6/11、HPV16/18的检测及意义

摘　　要：目的:探讨P21、ILK及HPV 6/11、HPV 16/18与外阴恶性肿瘤的关系。方法:P21、ILK用免疫组化SP法检测,HPV 6/11、HPV 16/18用原位杂交法(ISH)检测。结果:外阴恶性肿瘤组、外阴上皮内瘤变(VIN)组、正常组中,P21、ILK的阳性表达率分别为40.00%(12/30)、52.38%(11/21)、0(0/10)和63.33%(19/30)、47.62%(10/21)、0(0/10);外阴病变各组P21、ILK的阳性表达率均显著高于正常组(P<0.05)。外阴恶性肿瘤组、VIN组中HPV 6/11、HPV 16/18的阳性表达率分别为60.00%(18/30)、42.86%(9/21)和33.33%(10/30)、28.57%(6/21),正常对照组未检测出;HPV 6/11在外阴恶性肿瘤组、VIN组中的阳性表达率均显著高于正常组(P<0.05);HPV 16/18在外阴恶性肿瘤组中的阳性表达率显著高于正常组(P<0.05)。结论:P21、ILK及HPV感染在外阴恶性肿瘤的发生及发展中可能起一定的作用。Objective： To investigate the relationships between P21, ILK and HPV 6/11, HPV 16/18 and vulvar malignant tumor. Methods： P21 and ILK were stained immunohistochemieally, and HPV 6/11 as well as HPV 16/18 were detected by using in site hybridization method. Results： The expression rates of P21 and ILK in vulvar malignant tumor group, VIN group and normal group were 40.00% （12/30）, 52.38% （11/21）, 0（0/10） and 63.33% （19/30）, 47.62% （10/21）, 0 （0/10） respec tively, with significant difference among the vulvar lesions and normal skin epithelium （P〈0.05）. The positive rates of HPV 6/11, HPV 16/18 in vulvar malignant tumor group and VIN group were 60.00% （18/30）, 42.86% （9/21） and 33.33% （10/30）, 28.57% （6/21） respectively, and there was no HPV 6/11 or HPV 16/18 in normal skin epithelium. The positive rate of HPV 6/11 in vulvar lesions was significantly higher than that of the normal skin epithelium （P〈0.05）. Meanwhile, the positive rate of HPV 16/18 in vulvar malignant tumor was significantly higher than that of the normal skin epithelium （P〈0.05）. Conclusion： Over expression of P21, ILK and HPV 6/11, HPV 16/18 may play a role in the development and progress of vular malignant tumor.
