摘 要:目的:通过观察手术中一次性大剂量照射后不同时相兔直肠组织的病理变化和转化生长因子-β1(TGF-β1)含量的动态变化,以及IFN-γ早期干预的影响,探讨IFN-γ防治兔直肠组织大剂量照射后损伤的作用和机理。方法:雌性新西兰大白兔54只,体重(2000±200)g,随机分为单纯照射组(R组,25只),药物干预组(I组,25只),空白对照组(C组,4只)。手术中直视下用直线加速器给予R组和I组全盆腔一次性大剂量照射(20Gy),于术中照射后当天开始每隔4天给予I组肌注IFN-γ250000U/kg/f,共5次;R组、I组于术中照射后第4周,8周,12周,16周,20周分别处死5只兔(经耳缘静脉空气栓塞致死),C组于实验完成时处死,取直肠组织切片行原位杂交法测TGF-β1 mRNA比较不同时相各组直肠组织TGF-β1 mRNA的表达水平。结果:R组TGF-β1 mRNA于照射后第4周增高,第20周含量最多;I组与R组比较差异有显著性(P<0.05)。结论:给予一次性大剂量术中照射后,兔直肠组织TGF-β1 mRNA的表达随时间的推移而增多,而早期应用IFN-γ干预可明显抑制TGF-β1的表达,可减轻早期放射性炎症反应及后期放射性纤维化的程度。Objective: To observe the expression of transforming growth factorβ1 (TGF - β1 ) changes in rectum tissue of rabbits with introoperative radiotherapy(IORT) induced -injury for exploring the preventive effect of interferon- gamma( IFN- γ). Methods: Fifty four big albino rabbits were randomly allocated into the simple irradiation group (n =25) ,the IFN-γ treatment group (n =25) and the blank control group (n =4). The rectum of all rabbits except those in the blank control group were IORT,total of 20Gy. Five rabbits in the two groups were randomly sacrificed at each of the 5 time points (4,8,12,16, and 20 weeks after IORT), their rectum was harvested out, sliced and dyed with stain,hybridization in situ,then the levels of TGF - β1 mRNA expression in the rectum tissue were observed by hybridization in situ. Results : The most serious acute factitial proctitis was seen in the 4th week and rectal fibrosis was remarkable in the 20th week and the levels of TGF - β1 mRNA expression were enhanced in the 8th week, remarkable in the 16th week. While in the IFN -γ treatment group, the expressions of TGF -β1 mRNA at different time phase significantly lower than those in the simple irradiation group ( P 〈 0. 05 ). Conclusion: Given IORT,the levels of TGF - β1 mRNA expression in the rectum tissue were ., enhanced, IFN -γ/can abate acute factitial proctitis and rectal fibrosis, showing preventive and treating effect on radiation induced rectal fibrosis.
