多西紫杉醇不同给药方案治疗老年转移性乳腺癌临床观察

摘　　要：目的:比较多西紫杉醇每周方案及每3周方案治疗老年转移性乳腺癌的疗效与毒性。方法:41例老年转移性乳腺癌患者,随机分组后,分别接受不同方案多西紫杉醇治疗。多西紫杉醇3周方案组22例,剂量为75mg/m2/3周;每周方案组19例,每次剂量为25mg/m2×3周,休息1周。评估多西紫杉醇不同用药方案疗效与毒性。结果:单药多西紫杉醇治疗老年转移性乳腺癌的总有效率为34.1%。每周方案组与接受常规治疗方案组(多西紫杉醇每3周1次)的病人相比,有效率(RR)为36.8%vs 31.8%(P=0.504),两者比较差异无统计学意义;两组中Ⅲ-IV度骨髓抑制率为63.6%:31.6%(P<0.01)。但每周方案中III-IV度神经病变高于每3周方案组,为26.3%:18.2%。结论:单药多西紫杉醇治疗老年转移性乳腺癌有效。每周方案与每3周方案比较,有效率相近,但其血液学毒性低于每三周多西紫杉醇方案。每周方案对老年转移性乳腺癌患者是一较好的选择,可提高老年患者的生活质量。Objective：To compare the efficacy and toxicity of docetaxel as a single agent for treating elderly meta- static breast cancer with weekly and every 3 weekly scheme. Methods： Forty - one patients with elderly metastatic breast cancer received docetaxel as a single agent with different scheme. One group （22 patients ） with the dose 75mg/m^2 was given every three weeks. The other group （ 19 patients ）with the dose 25mg/m^2 given every week. Results： The Overall response rate （ORR） of docetaxel as a single agent was34.1%. The response rate （RR） of docetaxel were achieved 36.8% in the weekly administered of docetaxel group,compared with patients receiving standard therapy group （docetaxel every 3 weeks） ,the response rates was 31.8% ,there was no significant difference between the two groups（ P = 0. 504）. Grade 3 -4 WBC toxicity hematological toxicity rates in weekly and every 3 weekly docetaxel were 31.6% ,63.6% ,respectively（ P 〈 O. 01 ）. But grade 3, or 4 neuropathy was more frequent in weekly docetaxel than with docetaxel every 3 weeks （26.3% vs. 18.2% ）. Conclusion： Docetaxel is effective in treatment of elderly metastatic breast cancer, compared with patients receiving weekly docetaxel and standard therapy （docetaxel every 3 weeks）, the efficacy was similar, but grade 3 -4 hematological toxicity in weekly docetaxel group was relatively low. Weekly doeetaxel is an effective therapy for patients with elderly metastatic breast cancer, and anc improve the quality of life of patients.
