摘  要:目的探讨高海拔地区乳腺癌及乳腺良性病变中c-myc、HIF-1α、VEGF的表达与转移及预后的关系。方法应用免疫组化Elivision plus法,测定60例乳腺癌和20例乳腺良性病变中c-myc、HIF-1α、VEGF的表达。结果60例乳腺癌组织中c-myc、HIF-1α、VEGF阳性表达率分别为55.0%、51.7%、63.3%,与乳腺良性病变比较,差异有统计学意义(P<0.01)。c-myc的阳性表达与年龄、淋巴结转移有关(P<0.05)。HIF-1α、VEGF的阳性表达与组织学分级、临床分期及淋巴结转移有关(P<0.05)。HIF-1α、VEGF的表达呈正相关(r=0.371;P<0.01),与c-myc蛋白表达呈正相关(r=0.332;P<0.01),c-myc、VEGF的表达呈正相关(r=0.369;P<0.01)。结论c-myc、HIF-1α、VEGF在乳腺癌中的高表达与乳腺癌淋巴结转移密切相关。c-myc、HIF-1α、VEGF在促进乳腺癌淋巴结转移中可能起协同作用,联合检测乳腺癌中c-myc、HIF-1α、VEGF的表达,有利于更好地评估乳腺癌的生物学行为,提高对患者预后判断的准确性,指导个体化治疗。Objective To investigate the correlation among c-myc,HIF-1α and VEGF expressions in breast cancer and benign breast diseases;and to explore the correlation between metastasis and prognosis of breast cancer in high altitude area.Methods Elivision plus immunohistochemistry was employed to detect the expressions of c-myc,HIF-1α,VEGF in 60 cases of breast cancer and 20 cases of benign breast diseases.Results The expressions of c-myc,HIF-1α,VEGF in the 60 cases of breast cancer were 55.0%,51.7% and 63.3%,respectively,higher than those in benign breast diseases(P〈0.01).In the group of breast cancer the expression of c-myc was associated with age and axillary lymph node metastasis(P〈0.05),the expressions of HIF-1α,VEGF were associated with histological grading,axillary lymph node metastasis,and clinical stage(P〈0.05).The expression of HIF-1α was positively correlated with the expression of VEGF(r=0.371,P〈0.01),the expression of c-myc was positively correlated with the expression of HIF-1α(r=0.332,P〈0.01),the expression of c-myc was positively correlated with VEGF(r=0.369,P〈0.01).Conclusions The high expressions of c-myc,HIF-1α and VEGF are associated with axillary lymph node metastasis of breast cancer.The expressions of c-myc,HIF-1α and VEGF can synengisticly act to promote axillary lymph node metastasis in breast cancer.The combined detection of c-myc,HIF-1α and VEGF is helpful for evaluation of the biological behaviors of breast cancer,raise of accurate judgement of prognosis in breast cancer,direction of treatment personly.
