摘  要:目的研究高危型HPV-DNA染色体整合状态与宫颈上皮内瘤变及宫颈癌之间的关系。方法应用原位杂交和免疫组化方法检测95例宫颈上皮内瘤变(CINⅠ、CINⅡ、CINⅢ级分别21,18,16例)及宫颈癌组织中HPV感染情况,观察阳性杂交病例中HPV-DNA染色体整合状态与宫颈上皮内瘤变及宫颈癌关系。结果免疫组化检测CINⅠ、CINⅡ、CINⅢ级阳性率分别为14%,16.6%,37.5%,宫颈鳞癌阳性率46.6%(P<0.05);原位杂交检测CINⅠ、CINⅡ、CINⅢ级阳性率分别为19%,27.7%,37.5%,宫颈鳞癌阳性率70%(P<0.05);在CINⅠ、CINⅡ级中HPV-DNA以染色体外游离状态存在,在CINⅢ级及宫颈鳞癌中出现HPV-DNA整合状态。结论HPV-DNA染色体整合状态与宫颈上皮内瘤变及宫颈癌有关,在一定程度上HPV-DNA染色体整合状态对宫颈上皮内瘤变的恶性转归具有提示意义。Objective To explore the relationship of high risk HPV-DNA integration and cervical intraepithelial neoplasia(CIN). Methods HPV infection was detected in 95 cases of CIN and cervical cancer by immunohistochemistry and in situ hybridization, and HPV-DNA integration status was observed. Results HPV positive rates in CINⅠ、CINⅡ、CINⅢwere 14%, 16.6%, 37.5%, respectively ,lower than that in cervical cancer by immunohistochemistry staining(46.6%, P 〈 0.05 ). The positive rates in CINⅠ、CINⅡ、CINⅢ were 19% ,27.7% ,37.5 % by in situ hybridization, respectively, significantly lower than that in invasive cancer(70%, P 〈 0.05). HPV-DNA showed free body in CINⅠ、CINⅡ , and integrated body inCINⅢ and invasive cancer. Conclusion HPV- DNA integration status is related to CIN and cervical cancer,and might predict the prognosis of malignant CIN.
