经腹膜后途径腹腔镜肾囊肿去顶术

摘　　要：报告经腹膜后途径腹腔镜治疗肾囊肿６例共７侧，患者平均４７．８岁。左侧２例，右侧３例，双侧１例。囊肿部位下极３例，中部１例，上极２例，多发１例。囊肿最大直径平均６．４ｃｍ，平均手术时间１３５ｍｉｎ，平均术后住院５．８ｄ。该组除并发水囊破裂而致腹膜破裂，改腹腔镜辅助下开放手术１侧次外，余均成功。术后随访平均３．５月，无囊肿复发。作者对术式的适应证、操作要点、合并症产生原因及预防和处理进行了讨论，认为在有条件的医院，腹腔镜肾囊肿去顶术可作为一项常规术式，取代大部分开放去顶术。Six patients who underwent a total of 7 procedures of retroperitoneal laparoscopic cystectomy of renal cyst were reported.Mean patient age was 47.8 years old.The cysts were on the left in 2,on the right in 3 and bilateral in 1,located at the lower poles in 3,the middle portion in 1,the upper pole in 2 and being multiple in 1.The maximal diameter of the cysts was 6.4 cm in average.The mean operation time was 135 minutes.The average length of hospital stay was 5.8 days.Success has been achieved in all procedures except one with rupture of balloon dilator,which resulted in perforation of peritoneum and laparoscopy-assisted cystectomy had to be done.No recurrence has been found in all patients at a mean follow-up of 3.5 months.The indication,method,and complication of the procedure have been discussed.In conclusion,retroperitoneal laparoscopic cystectomy of renal cyst is a safe and effective alternative to open surgical techniques.
