晚期胃癌患者外周血CEA mRNA的表达及临床意义

摘　　要：目的探讨晚期胃癌患者外周血中CEA mRNA的表达及其临床意义。方法将临床确诊的32例晚期胃癌根据影像学检查分为血行转移组13例,局部浸润组19例,对照组为胃良性病变10例。以外周血为检测标本,用反转录巢式聚合酶链反应技术(RT-NP-PCR)检测CEA mRNA。结果初诊时对照组阳性率0%,局部浸润组阳性率63.16%,血行转移组阳性率92.31%,两胃癌组与对照组比较差异有统计学意义(P<0.01),两胃癌组比较差异无统计学意义(P>0.05);6个月后无血行转移病例阳性率50.00%,有血行转移病例阳性率90.00%,两者比较差异有统计学意义(P<0.05)。结论应用RT-NP-PCR技术检测晚期胃癌患者外周血CEA mRNA可作为晚期胃癌血源性播散的指标,对指导选择治疗方案和判断预后有重要意义。Objective To evaluate the clinical significance of the expressions of CEA mRNA in peripheral blood of patients with advanced gastric carcinoma. Methods 32 patients with advanced gastric carcinoma, were diagnosed by tomography and pathology, were divided into two groups： hematogenous metastases group （n = 13） , local infiltration group （n = 19）. 10 patients with chronic gastritis or gastric ulcer were regarded as the control group. RT-NP-PCR was used to detect CEA mRNA in peripheral blood. Results In the control group, the detected results were all negative. In the gastric carcinoma group, the cases with positive results of CEA mRNA in the hematogenous metastases group were 12 （92.31%） , and in the local infiltration group were 12 （63.16%）. There was significant difference between the two gastrie carcinoma group and the control group. There was no significant difference between the two gastric carcinoma groups （P 〈0. 05）. However, after six months, the cases with positive results of CEA mRNA in patients who had the hematogenous metastases was 18 （90%） , and in the patients who had no hematogenous metastases was 6 （ 50.00% ）. Significant difference was indicated in the two groups （ P 〈 0.05）. Conclusion CEA mRNA detected by RT-NP-PCR is a useful index to diagnose the hematogenous metastases in the patients with advanced gastric carcinoma. It can be helpful to choose the treatment options and evaluate the prognosis of advanced gastric carcinoma.
