同步放化疗或单纯放疗治疗食管癌的疗效观察

摘　　要：目的观察3DCRT联合同步化疗或单纯3DCRT治疗食管癌的近期有效率、局部控制率、生存率。方法89例食管癌分为3组。同步放化疗组（A组）：给予局部3DCRT，DT60Gy，放疗第1、5周给予DDP＋5-Fu方案化疗，放疗结束1～2月后巩固化疗3—4周期；3DCRT组（B组）：给予局部3DCRT，DT60～70Gy；常规放疗组（c组）：给予常规放疗DT68～72Gy。结果3组有效率分别为92．86％、83．87％和80．00％（P〉0．05）。1年控制率为82．14％、77．42％和63．33％（P〉0．05）；1年生存率为75．00％、67．74％和60．00％（P〉0．05）；2年控制率为71．43％、64．52％和46．67％（P〈0．05）；2年生存率为64．29％、51．62％和43．33％（P〉0．05）；3年控制率为60．71％、45，16％和30．00％（P〈0，05）；3年生存率为46．43％、41．94％和23．33％（P〈0．05）。结论3DCRT联合同步化疗或单纯3DCRT治疗食管癌比常规放疗疗效好，3DCRT联合同步化疗比单纯3DCRT疗效好。Objective To investigate the effective power, local control rate and survival rate of 3DCRT or 3DCRT combined with concurrent chemotherapy in the treatment of patients with esophageal carcinoma. Methods We divided 89 patients with esophageal carcinoma into 3 groups. Concurrent chemoradiotherapy group （A） ： using 3DCRT, DT 60 Gy, meanwhile giving chemotherapy with DDP ＋ 5-Fu at the first and the fifth week. 1-2 months later, to consolidate therapeutic effect by the same chemotherapy. 3DCRT group （B） ： using 3DCRT, DT 60- 70 Gy. Conventional radiotheraphy group （C） ： using conventional radiotherapy, DT 68-72 Gy. Results The effective power of A, B,C three groups was 92.86%, 83.87%, 80.00% , respectively （ P 〉 0.05 ） ; the one year control rate is 82.14%, 77.42%, 63.33% , respectively （ P 〉 0.05 ） ; the two year control rate was 71.43 %, 64.52%, 46.67 % , respectively （ P 〈 0.05 ） ; the three year control rate was 60.71% , 45.16%, 30.00%, respectively（ P 〈 0.05） ; the survival rate of the one year was 75.00%, 67.74%, 60.00% , respectively（ P 〉 0.05） ; the survival rate of the two year was 64.29%, 51.62%, 43.33% , respectively（P 〉 0.05 ） ; the survival rate of the three years was 46.43% , 41.94%, 23.33% , respectively （ P 〈 0.05）. Conclusion 3 DCRT combined with concurrent chemotherapy and 3 DCRT has better efficacy, as compared with the conventional radiotheraphy; 3DCRT combined with concurrent chemotherapy has better efficacy, as compared with 3DCRT.
