异丙酚对癫痫病灶切除中皮层脑电图的影响

摘　　要：目的：观察异丙酚不同剂量及血药浓度对癫痫病人术中ＥＣＯＧ的影响。方法：２０例癫痫手术病人先用静吸复合麻醉，开颅后排出原吸的麻醉药，当异氟醚呼出浓度降至０．２％时，描记ＥＣＯＧ记录癫痫波，微量泵先后静注异丙酚３ｍｇ·ｋｇ－１·ｈ－１、５ｍｇ·ｋｇ－１·ｈ－１、７ｍｇ·ｋｇ－１·ｈ－１，同时观察ＥＣＯＧ变化，测定不同剂量异丙酚血药浓度。结果：异丙酚静注３ｍｇ·ｋｇ－１·ｈ－１、５ｍｇ·ｋｇ－１·ｈ－１、７ｍｇ·ｋｇ－１·ｈ－１后血药浓度分别为０．８７±０．０９ｍｇ／Ｌ、１．４２±０．２９ｍｇ／Ｌ、２．４１±０．２０ｍｇ／Ｌ。ＥＣＯＧ和用药前比，α波频率稍增快，癫痫波没有完全被抑制。结论：异丙酚是癫痫手术病人麻醉的良好选择。Objective:To investigate the effects of different doses of propofol on ECOG in patients undergoing epileptic foci resection. Methods: 20 epileptic patients were anesthetized with isoflurane inhalation supplemented with iv. fentanyl and vecuronium. While the brain cortex exposed, isoflurane was discontinued and washed out intentionally and its expiratory concentration decreased to <0 2%. Propofol in a dose of 3mg·kg -1 ·h -1 ,5mg·kg -1 ·h -1 ,7mg·kg -1 ·h -1 was infused iv. by micropump in succession. Meanwhile, ECOG was recorded and plasma propofol concentration was measured. Results:Blood concentrations of propofol were 0 87±0 09,1 42±0 29,2 41±0 2mg/L respectively in correspordane with the different dosages of iv. propofol. The frequency of α wave was somewhat accelerated and the epileptic spike wave complexes were retained. Conclusion:propofol can be used appropriately for the epileptic foci resection patients.
