肥胖对哈萨克族和汉族正常血压组和高血压组左室重构及左心功能的影响

摘　　要：目的旨在探讨肥胖对新疆哈萨克族(哈族)和汉族正常血组组和高血压组左室重构和左心功能的影响。方法选取新疆昌吉地区哈族和汉族人群共550例,其中高血压组(EH)238例,正常血压组(NT)312例,根据亚太地区成人肥胖的界定标准(BMI≥25 kg/m^2),将所有受试对象分为肥胖组和非肥胖组,常规测量左室心肌质量(LVM)、左室心肌质量指数(LVMI)和E/A等超声指标。结果在NT组和EH组中,肥胖组的LVM、LVMI均高于非肥胖组。左室重构检出率也较非肥胖组增高,但E/A低于非肥胖组;哈汉两民族内,肥胖组的LVM、LVMI亦高于非肥胖组,但E/A低于非肥胖组。结论肥胖对新疆哈、汉两民族正常血压组和高血压组左室重构均具有影响,可引起左室舒张功能障碍。Objective To evaluate the effects of obesity on left ventricular reconstruction and left ventricular function in normotensives and hypertensives of Kazaks and Hans in Xinjiang. Methods Left ventricular mass （LVM）, left ventricular mass index （LVMI）, E peak and A peak at the mitral annulus were acquired by echocardiography in 550 hypertensives and normotensives of Kazaks and Hans of Changji in Xinjiang among which there were 238 hy pertensives and 312 normotensives. According to the definition of abdominal obesity： BMI≥25 kg/m^2 , the subjects were divided into the obese group and non-obese group. Results In both normotensives and hypertensives group, LVM and LVMI were higher and E/A indexes decreased from obese group than those from non-obese group, and the same results were observed in each race group separately. The incidence of left ventricular reconstruction also increased in obese group. Conclusions Obesity had an effect on left ventricular reconstruction and decreased left ventricular diastolic function in normotensives and hypertensives of Kazaks and Hans in Xinjiang.
