鸟氨酸氨基甲酰转移酶速率法测定及其在肝病中的诊断意义

摘　　要：目的采用速率法测定鸟氨酸氨基甲酰转移酶活性在各型肝病中的诊断价值。方法在pH=7.2,1/10mol/L磷酸盐缓冲条件下,以瓜氨酸为底物,经一系列酶促反应,在全自动生化分析仪上测定鸟氨酸氨基甲酰转移酶的活性。结果鸟氨酸氨基甲酰转移酶在急性病毒性肝炎、肝性脑病时显著升高,与ALT、AST、GGT呈正相关;在慢性肝炎、肝硬化、肝癌时中度升高,阳性率高达90%以上;酒精性肝炎、药物性肝炎时轻度升高。结论鸟氨酸氨基甲酰转移酶是反映肝损伤的灵敏且特异的理想指标,采用速率法测定其活性简便、稳定、准确、快速,尤其可以作为急性病毒性肝炎早期和慢性肝病晚期的常规检测指标。Objective To determine the diagnostic value of Ornithine carbamoyltransferase in hepatic diseases.Methods In optimal alkaline phosphate buffer,Citrulline was used as substrate and added to a sort of enzymatic reactions.Serum OCTactivity of healthy adult and hepatopathy was determined by the rate of speed method with automatic biochemistry analyzer.Results The activity of OCT was significantly higher in acute viral hepatitis and hepatic encephalopathy patients.There was a statistically significant correlation between OCT and serum ALT,AST,and GGT.In chronic hepatitis,liver cirrhosis and hepatoma,the value of OCT was medium to high with at least 90% positivity rate.OCT activity of alcoholic hepatitis and drug induced hepatitis patients is slightly high.Conclusion OCT is a sensitive and specific index to reflect hepatic injury.It can be detected by the rate of speed method which is accurate,easy and provide results simply.It can also be applied as a routine monitoring index for early acute viral hepatitis and late chronic liver disease.
