成人型肺母细胞瘤临床病理分析

摘　　要：目的:探讨成人型肺母细胞瘤的临床病理特征、免疫组织化学特点及鉴别诊断。方法:报道1例成人型肺母细胞瘤并结合文献复习,进行病理组织学和免疫组织化学标记的观察及随访。结果:1例成人型肺母细胞瘤病理形态学特点为由不同比例、不成熟的间叶和(或)分化良好的恶性上皮成分组成;免疫组织化学染色上皮细胞CK、EMA、syn(少部分)、CgA(少部分)等阳性,而间叶细胞Vimentin、S-100(软骨分化岛和少部分上皮)、CD117(部分)阳性,AFP上皮细胞和间叶细胞均灶(+);其余如desmin、HMB45、MC、TT-F1、Bcl-2及P53等均阴性。结论:成人型肺母细胞瘤是肺原发性恶性度极高的肿瘤,需与另一类型肺母细胞瘤-胸膜肺母细胞瘤、肺癌肉瘤、原发性肺腺癌及畸胎癌等恶性肿瘤鉴别。Objective：To investigate the clinicopathological and immunohistochemical features,differential diagnosis of adult type pulmonary blastoma（PB）.Method： Pathomorphological study was made on one case of adult type pulmonary blastoma,and relative literature review were made to summarize Pathomorphological study,immunohistochemical mark,prognosis.Results： The case of adult type pulmonary blastoma was consist of different proposition,immunature and/or well-differentiated malignant epithelial component.Immunohistochemical studies showed CK、EMA、syn（a little）、CgA（a little）were positive in epithelial component,Vimentin,S-100（nodules of cartilage and a little epithelial component）,CD117（partly） was positive in the stromal component,AFP was positive in epithelial and stromal components;desmin、HMB45、MC、TTF-1 Bcl-2 and P53 were negative.Conclusion： The adult type pulmonary blastoma is a high-grade primary malignant tumor with poor-prognosis,and it should be distinguished from another type pulmonary blastoma-pleuropulmonary blastoma,carcinosarcoma,primary pulmonary adenocarcinoma and,teratocarcinoma.
