直肠全系膜切除术治疗中下段直肠癌48例临床体会

摘　　要：为探讨直肠全系膜切除术（TME）在直肠癌根治术中的应用技巧和注意事项，回顾分析48例行TME的中下段直肠癌患者的病例资料。结果显示，术后随访1～22个月，无吻合口漏及局部复发；肝转移2例，死亡1例。手术时间75～115min，平均90min；术中出血量110～280ml，平均180ml。结果表明，TME能有效地降低直肠癌患者术后的局部复发率，是直肠癌术中必须遵循的原则之一。The clinical data of 48 patients with middle and lower rectal carcinoma who had undergone total mesorectal exeision（TME） was analyzed in an attempt to review the surgical skills and points for atten- tion. As results,follow up was conducted for 1-22 months,there was no anastomotic leakage or local recur- rence,hepatometastasis was found in 2 cases and death in 1 case. The operative time was 75-115 minutes, averaged 90 minutes, the intraoperative blood loss was 110-280ml, averaged 180ml. It is concluded that TME can ensure a reduction of local recurrence and serves as one of the vital principle in the surgical treat- ment of rectal carcinoma.
