摘  要:目的探讨18F-FDG PET脑显像像素统计参数图(SPM)法在颞叶内侧癫痫术前定位中的价值。方法对17例颞叶内侧癫痫患者行脑18F-FDG PET检查,对其所有PET图像进行感兴趣区(ROI)法及SPM分析法比较。SPM分析法所用健康对照组选自同期于本中心健康查体者20例。结果ROI法对癫痫灶准确定侧率为70.6%,准确定位率为29.4%;SPM法分别为70.6%、47.1%。二者准确定位率比较,P<0.05。SPM法发现双侧颞叶代谢异常9例(52.9%),伴额、顶叶皮层代谢异常者12例(70.6%)。结论18F-FDG PET脑显像SPM法是一种简便、客观、有效的颞叶癫痫灶定位方法,且有助于发现脑代谢异常。Objective To evaluate the value of 18F-FDG PET using statistical parametric mapping SPM in the preop- erative localization of mesial temporal lobe epilepsy. Methods Seventeen patients with mesial temporal lobe epilepsy were examined with 18 F-FDG PET, all the PET imaging were analyzed by region of interest (ROI) method and SPM analysis method. Twenty healthy controls were chosen as the control group for SPM analysis. Results The rate of accurate determi- nation of the side and accurate localization were 70.6% and 29.4% for the ROI method , the SPM method were 70.6% and 47.1%, there was significant difference of accurate localization rate between the two methods, P 〈 0.05. The SPM anal- ysis revealed nine (52.9 % )patients with bilateral temporal metabolic abnormality, twelve( 70.6% )patients with frontal and parietal cortex abnormality. Conclusions The 18 F-FDG PET using SPM is an objective, convenient and effective method for the localization of mesial temporal lobe epilepsy, and may help to find abnormal cerebral metabolism.
