摘 要:目的探讨颅内血肿微创清除术治疗高血压脑出血的临床疗效。方法51例脑出血患者,男性36例,女性15例,均有高压病史。基底节区出血45例(28~90ml),皮质下出血6例(25~55ml),其中有11例破入脑室。根据CT定位,应用YL-1型一次性颅内血肿粉碎性穿刺针穿刺抽吸血肿,术后辅以尿激酶纤溶治疗。结果穿刺引流时间18h~5d。51例患者,术后血肿完全清除19例,清除80%以上27例,清除60%~80%5例。结论颅内血肿微创清除术治疗简单、安全、创伤小,能较快清除血肿,解除占位效应,提高患者生存质量,降低致残率及病死率。Objective To investigate the clinical effect of minimally invasive removal of hypertensive intracerebral hemorrhage, Methods The group of 51 cases of cerebral hemorrhage patients were male and 36 cases, female 15 cases, have history of hypertension Basal ganglia hemorrhage 45 cases(28-90 ml) ,subcortical hemorrhage in 6 cases(25-55 ml) ,of which 11 cases of breaking into the ventricle. The puncture point of all patients were determined by CT scan and a marker was made there, then the skull was drilled with YL-1 type of intracranial hematoma puncture needle and the hematoma was sucked out followed by Urokinase fibrinolytic treatment after operation. Results The drainage lasted from 18 hours to 5 days. Among 51 cases under this treatment, the hematoma was removed completely in 19 patients, more than 80% was removed in 27 patients and 60% - 80% was removed in 5 patients. Conclusion Minimally invasive drainage was simple, safe and microtraumatic. It can also remove the hematoma efficiently, relieve the space-occupying effect, improve patient quality of life and reduce morbidity and mortality.
