卒中后单侧空间忽略综合康复训练疗效的研究

摘　　要：目的探讨综合康复训练对卒中后单侧空间忽略(Unilateral spatial neglect,USN)患者运动功能和日常生活能力(ADL)康复的影响。方法选取伴有USN的脑卒中患者60例,将其随机分为治疗组和观察组,同时选取不伴有USN脑卒中患者30例为对照组,三组患者均采用常规康复措施进行治疗,治疗组同时接受综合康复训练。临床疗效评定采用Fugl-Meyer运动功能评分法,日常生活活动能力(ADL)评定采用改良Barthel指数(MBI)。结果3组患者分别经治疗后,其Fugl-Meyer运动功能评分和MBI评分均较治疗前明显改善(P<0.01),观察组Fugl-Meyer运动功能评分和BI评分均明显低于治疗组和对照组水平(P<0.01);而治疗组和对照组上述指标比较,差异无统计学意义(P>0.05)。结论单侧空间忽略明显影响脑卒中患者的康复治疗效果,对伴有USN的脑卒中患者实施综合康复训练,不仅提高患者运动功能,而且对ADL也有显著促进作用。Objective To explore the effects of comprehensive rehabilitation on the motor function and activities of daily living （ADL） in patients with unilateral spatial neglect（USN） after stroke. Methods Sixty patients with USN were randomly divided into treatment group and observation group, and 30 patients without USN after stoke were as control. Three groups were treated with routine rehabilitation measures, while the treatment group with additional comprehensive rehabilitation. They were evaluated with Fugl-Meyer motor assessment（FMA） scale for the function and with Barthel-index （MBI） for ADL before and after treatment. Results FMA and MBI scores of patients in three groups were increased significantly （ P 〈 0.01 ） after treatment. The scores of the observation group were significantly lower than that of the control group and treatment group（ P 〈0.01 ）. There was no significant difference between the treatment group and the control group （ P 〉0. 05）. Conclusion The USN significantly affects the rehabilitation effect, and the comprehensive rehabilitation can improve the motor function and ADL.
