开患者固定矫治配合肌功能训练临床疗效观察

摘　　要：目的探讨肌功能训练在正畸治疗中的作用。方法将2007年7月在山西医科大学附属口腔医院正畸科就诊的处于生长发育期的60例骨性开患者,随机分为对照组30例,用MEAW技术矫治;试验组30例,矫治同时配合相应的肌功能训练。比较两组患者在排齐整平、关闭拔牙间隙、精细调整3个阶段中矫治效果的差异。结果(1)排齐整平阶段:对照组有6例出现了开加重现象,实验组无一例出现开加重现象,比较此过程中两组垂直开度变化情况,差异具有统计学意义(P<0.01)。(2)关闭拔牙间隙阶段:试验组上下颌第一磨牙区牙槽骨高度均较矫治前有所降低,前下面高/后下面高(ALFH/PLFH)值较矫治前减小,AB平面与下颌平面的交角、腭平面与眼耳平面的交角之和(ODI)值有所增大,下颌平面角(GoGn-SN)减小;对照组各指标与实验组相比均变化较小。(3)精细调整阶段:两组间各项指标的治疗前后变化差异均具有统计学意义(P<0.05)。结论开畸形患者矫治早期即进行肌功能训练有助于提高正畸治疗的疗效。Objective To investigate the clinical effect of functional exercises for muscles during orthodontics treatment. Methods A total of 60 cases of skeletal openbite patients who visited Orthodontics Department of Dental Hospital attached to Shanxi Medical University in July 2007 were selected during their growth stage. The patients were divided into 2 groups of 30 people each randomly, and all of the samples were treated with MEAW technique. The cases of the test group accepted the functional exercises for muscles at the same time, and compare the treatment effects of each groupduring the aligning and leveling stage, in closing the extraction space and delicate adjustment stage. Results （ 1 ） During the aligning and leveling stage, 6 samples have shown aggravation of openbite in the control group, but the test group was without this phenomenon , compared with vertical openbite during this stage . There was statistical significance between the two groups （P 〈 0.01 ） （2）In closing extraction space stage, in the test group, in the first molars, alveolar heights of maxillary and mandibular were lower than the early stage ; ALFH / PLFH ratios were decreased ; ODI were increased; GoGn-SN angles were decreased. The variety of indicators in the control group was less than the test group. （3） In the delicate adjustment stage, the variety of indicators have statistical significance （P 〈 0.05）in the two groups Conclusion The effect of orthodontics treatment for openbite patients is improved with functional exercises for muscles during the early stage.
