随访护理指导对直肠癌超低位保肛术后排便功能恢复的影响

摘　　要：目的探讨以随访护理指导模式,对超低位直肠癌行括约肌间切除手术(ISR)后的患者肛门排便功能康复的干预,评价随访护理指导模式的实用性和可行性。方法60例施行ISR手术的超低位直肠癌患者,按Williams的排便自制标准,分别于患者出院后的3、6、12个月分阶段实施护理干预,通过护理随访提供康复指导,收集患者社区功能恢复的情况,评价患者排便功能的康复情况。结果通过护理随访干预,本组经过术后3、6、12个月分别评价,有86.65%、93.33%、96.74%的患者达到排便功能良好的效果。结论根据对患者排便功能的评估情况,制定分阶段的社区护理干预措施,采用随访护理指导模式,同样能有效促使患者在社区遵医进行功能康复,随访护理指导在促进患者肛门排便功能恢复的同时,减少患者住院时间和经济负担,对促进患者尽早回归社会、提高病人的生活质量有重要的意义。Objective To estimate the pattern of follow - up nursing guide intervening in course of rehabilitation of defecation function after intersphincter resection（ISR） for the ultra - low rectal cancer and evaluate the prac- ticability and feasibility of the pattern of follow - up nursing guide. Methods According to the defecation of Wil- liams＇ standard, sixty ultra - low rectal cancer patients underwent the ISR were given community intervention with follow - up nursing guide in different stages. The rehabilitation of defecation functions were evaluated after 3 month,6 month and 12 month. Results Through the follow - up nursing guide, 86.65%, 93.33 % ,96.74 % of the patients received good effect in defecation function after 3 month, 6 month and 12 month respectively. Conclusion According to the patients＇ defecation function, formulate intervening measure of community care through staged and usage of the pattern of follow - up nursing guide also can promote the patients＇ rehabilitation of defecation function effectively in the community. The follow - up nursing guide can not only promote the rehabilitation of patients＇ anal defecation function, but also reduce the patients＇ hospital stays and eeonomic burden. It has a significant effect on improving the life qualities of these patients.
