LDH和β_2-MG及CA125测定对非霍奇金淋巴瘤预后判断的价值

摘　　要：为了定量测定非霍奇金淋巴瘤(NHL)中血清乳酸脱氢酶(LDH)、β2微球蛋白(β2-MG)及糖类抗原125(CA125)的临床意义,回顾分析65例经病理确诊的NHL患者的LDH、β2-MG及CA125,判断其与临床分期、治疗效果及预后的关系。结果:Ⅰ+Ⅱ期与Ⅲ+Ⅳ期,无症状组与有症状组、无骨髓浸润者与有骨髓浸润者相比,后者LDH和β2-MG均明显高于前者,差异有统计学意义,P<0.05。有浆膜腔积液及结外侵犯者,LDH和CA125明显高于无浆膜腔积液及结外侵犯者,经化疗缓解患者三者水平显著下降,P<0.05。LDH增高组与LDH正常组的2年生存率分别为90.1%和88.9%,差异无统计学意义,P>0.05;4年生存率分别为84.8%和25.2%,差异有统计学意义,P<0.05。CA125增高组与CA125正常组的2年生存率分别为87.6%和51.5%,差异有统计学意义,P<0.05。初步研究结果提示,血清LDH、β2-MG及CA125可能对NHL患者分期、预后及疗效判断有参考价值。This study was to evaluate the clinical significance of quantitative determination of serum lactic dehydrogenese （LDH）,beta-2 microglobulin （β2-MG） and CA125 in non-Hodgkin lymphoma （NHL）. The data of 65 patients with NHL from 2003 to 2004 were retrospectively analysed,the relationships between LDH,β2-MG and CA125 activities and the tumor phase,effect of treatment as well as prognosis were studied. Results：The values of LDH and β2-MG for patients in the advanced stage,B group and whose bonemarrow existed NHL cells were obviously higher than those in their comparison counterparts and the difference was significanct （P0.05）. A higher levels of LDH and CAl25 were associated with the effusions and extranodal involvement （P0.05）. The patients of elevated levels LDH,β2-MG and CA125 had less sensitivity to the treatment. There was no significient difference between the group of elevated levels of LDH and the group of normal levels in the two year survival rate （90.1% vs 88.9%）（P0.05）,but the difference in the four year survival rate was significant（84.8% vs 25.2%）（P0.05）. There was a significient difference between the group of elevated levels of CA125 and the group of normal levels in the two year survival rate （87.6% vs 51.5%）（P0.05）. In conclusion,the levels of LDH,β2-MG and CA125 are useful for the judgment of severity,response to treatment and prognosis of NHL.
